Requirements
  • A Bachelor or Master of Science degree in Occupational Therapy.
  • A current license or certification in the state of practice.
  • Certification, if applicable, and demonstrated competency in identified areas of specialization.
  • Excellent oral and written communication and interpersonal skills.
  • Demonstrated ability to assess resident needs and develop and implement a comprehensive plan of care.
  • Knowledge of accreditation standards and compliance requirements.
  • Ability to safely stand, push or pull up to 35 pounds, lift up to 50 pounds independently or heavier loads with assistance or a mechanical lift, stoop, reach, and balance, including for prolonged periods.
  • Knowledge of OBRA regulations, HIPAA, Local Coverage Determination Policy, and third-party reimbursement guidelines.
Responsibilities
  • Evaluate and treat residents in accordance with physician orders using therapeutic procedures and assistive devices to maximize functional potential.
  • Review resident information and evaluate its impact on the assessment and treatment process.
  • Conduct and document thorough functional assessments using standardized procedures, including home, vocational, and equipment needs.
  • Develop and implement individualized treatment plans with achievable functional goals.
  • Comply with evaluation, treatment, and documentation guidelines and maintain timely and accurate departmental records.
  • Enter a progress note in the resident record after each treatment session.
  • Revise treatment plans as needed throughout treatment.
  • Document the treatment course in a clear, concise discharge summary using the prescribed format.
  • Instruct assistants in treatment procedures and provide close supervision while they work with residents.
  • Orient residents to program services and treatment procedures and inform them of potential risks.
  • Work with nursing staff, residents, and families to maximize residents' functional abilities.
  • Teach residents and families or other caregivers exercises and therapeutic procedures to continue after discharge.
  • Assist with referrals when residents require additional services after discharge and maintain current knowledge of community resources.
  • Attend rehabilitation team conferences and provide information regarding resident progress, problems, or needs.
  • Attend department and facility meetings as required.
  • Pursue continuing education appropriate to the discipline and responsibilities to maintain clinical currency.
  • Participate in quality improvement by responding to medical-record audits, satisfaction surveys, safety activities, and regulatory requirements.
  • Provide input to the Rehabilitation Manager on assistants' clinical performance when requested and appropriate.
  • Promote Heritage Healthcare's programs and services through formal and informal community interactions.
  • Follow company and facility policies and procedures and participate in facility quality improvement and safety programs.
  • Perform other duties as assigned to provide quality resident care.
  • Understand reimbursement by various payers and allocate therapy resources efficiently according to resident goals, outcomes, and discharge destination.
  • Perform duties independently, recognize when direct supervision is needed, and assist willingly.
  • Maintain the productivity level established for the position at the applicable facility.
