Requirements
  • Successful completion of a Licensed Practical Nurse program from an accredited school.
  • Basic Life Support certification.
  • New York State Education Department Licensed Practical Nurse license.
  • Bilingual proficiency in English and Spanish.
Responsibilities
  • Provide quality nursing care in the department as directed by the departmental lead nurse.
  • Perform screening procedures and document them in accordance with organizational policies and procedures.
  • Identify potential patient problems, alert the registered nurse or provider, and anticipate orders for intervention.
  • Assist providers with in-office procedures and examinations as needed.
  • Participate in the care team model, including pre-visit planning.
  • Lead care-team huddles and communication.
  • Direct population-management activities for the assigned patient panel.
  • Direct and participate in quality-improvement activities within the care team and department.
  • Administer medications, treatments, and vaccines according to provider orders and scope of practice.
  • Perform hearing and vision screenings on children and adults.
  • Carry out medical-provider orders for electrocardiograms, emergency response, medication administration, vaccinations, prescribed treatments, and other procedures.
  • Adhere to safe nursing practices, including infection control, National Patient Safety Goals, medication management, and waived testing requirements.
  • Refer phone calls and patient triage to the lead nurse or physician.
  • Float to other departments and the laboratory as needed.
  • Make recommendations to supervisors for improving health care for patients.
  • Maintain a clean and safe patient environment.
  • Follow all clinical, administrative, and general organizational policies and procedures.
  • Attend and participate in monthly department, all-staff, and other required meetings.
  • Maintain confidentiality of patient, financial, and employee-relations information.
  • Perform other assigned duties.
