Requirements
  • An active California Licensed Vocational Nurse license is required.
  • Willingness and ability to work Monday through Friday from 8:30 a.m. to 5:00 p.m. in both field and remote settings.
  • At least two years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
  • Working knowledge of government and community resources related to social determinants of health.
  • Valid driver's license and reliable transportation.
  • General computer skills and working knowledge of Google Workspace, Microsoft Office, and the internet.
  • Employees in patient-, client-, or customer-facing roles must be vaccinated against influenza, subject to consideration of religious or medical accommodations.
  • Employment eligibility must be verified through E-Verify upon hire.
Responsibilities
  • Assess member needs in physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports.
  • Oversee the development of client care plans and goal setting.
  • Offer services where the member resides, seeks care, or finds most accessible, including office-based, telehealth, or field-based services.
  • Connect clients to needed social services and supports.
  • Advocate for clients with health care professionals and other care-team participants.
  • Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
  • Conduct outreach and engagement activities to facilitate linkage to the Enhanced Care Management program and log activity in the Client Relationship Management system.
  • Evaluate client progress and update SMART goals.
  • Provide mental health promotion.
  • Arrange transportation, including ACCESS services.
  • Complete documentation, including outcome measures, within the timeframes established by individual care plans.
  • Maintain up-to-date patient health records in the Electronic Medical Record system and other business systems.
  • Complete monthly reporting to ensure program compliance.
  • Attend assigned training.
