PATH is urgently seeking experienced social workers with experience working with vulnerable populations to join our PSH Program as a Clinical Case Manager.
ABOUT VILLAS AT FOURTH STREET
Villas at Fourth Street is a permanent supportive housing (PSH) development which provides seniors 55 and above who have experienced chronic homelessness with permanent housing and supportive services. By centralizing services on-site, PSH developments ensure ease of access for residents to needed resources and services. Services provided by the program are person-centered, culturally competent and designed to promote long-term housing stability, improve residents’ quality of life, and help them integrate into the community.
WHAT IS A CLINICAL CASE MANAGER?
Clinical Case Managers alongside a team of other case managers, housing locators and community providers, the Case Manager provides field-based, direct client outreach, linkage and retention services to people experiencing homelessness. Case management services aim to promote recovery and community integration and ultimately help clients break the cycle of homelessness. .
Responsibilities include:
- Perform initial comprehensive biopsychosocial assessment; develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services.
- Collaborate with the client to develop an individualized treatment plan that addresses short- and long-term goals.
- Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted.
- Provide field-based therapeutic supportive case management services addressing mental health, functional, and substance use barriers as needed to help clients obtain and maintain housing.
- Consult with LCSW and supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions, complete critical incident reporting as required per contract and PATH policies.
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage their own care.
- Provide coverage to the 24/7 After Hours Response phone line.

WHAT YOU BRING
We’re looking for candidates that:
- Maintain and execute confidential information according to HIPAA standards
- Demonstrate knowledge or experience with advanced case management techniques including critical time intervention, trauma informed care, harm reduction strategies, motivational interviewing, and crisis intervention techniques
- Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
- Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
PREFERRED QUALIFICATIONS
- Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW).
MINIMUM QUALIFICATIONS
- Master’s Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT)
- One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations.

- Ability to provide in-home and community-based services on a regular basis.
- Ability, willingness, and availability to work flexible hours which may include evenings or weekends to support the needs of the program.
- Ability to obtain CPR/First Aid certification through PATH.

- Proof of insurance and ownership for personal vehicles used during work duties
