- Licensed Practical Nurse (LPN) with current license
- Able to travel throughout the service area to patients' homes
- Available for full-time schedule Monday through Friday 8am-5pm
- Proficiency with electronic medical record documentation or willingness to document in EMR using company-issued laptops
- Must be able to check email intermittently to respond to documentation requests
- Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments
- Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans
- Educate patients and families on health management, medications, and self-care techniques
- Collaborate with the healthcare team to ensure the highest quality care for each patient
- Perform initial assessment and case development of patients in all stages of life in their homes, including assessing, interpreting, planning, implementing and evaluating patient care
- Document patient care in EMR and maintain accurate records in the home setting
- Travel to patients’ homes within the Guilford County area and surrounding regions to provide services
- Strong assessment, skilled nursing and documentation skills are strongly encouraged to apply
- Experience providing home health nursing services and case management would be advantageous
- Ability to work independently and manage a caseload in a home health environment
