Requirements
  • Graduate of an accredited school of nursing or educational equivalency for licensing.
  • One year of licensed practical nurse experience.
  • Basic Life Support HealthCare Provider certification from the American Heart Association.
  • Licensed Practical Nurse licensure issued by the Illinois Department of Financial and Professional Regulation for work in Illinois, or issued by a compact state for work in Missouri, Oklahoma, or Wisconsin.
Responsibilities
  • Deliver care to assigned clients and perform direct care in accordance with an established plan of care.
  • Deliver safe care in line with physician orders and established nursing care standards.
  • Maintain communication regarding patient progress with physicians, supervisors, staff, and family members as appropriate, and report patient-related observations to the supervising Registered Nurse daily.
  • Administer medications and observe patients for adverse reactions to medications or treatments.
  • Provide documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements, and ensures reimbursement.
  • Perform point-of-care testing according to policies and procedures when required.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
  • Use speech continuously to share information through oral communication.
  • Stand and walk continuously.
  • Lift, carry, push, or pull objects weighing 0–25 pounds frequently.
  • Sit, reach, and use a keyboard for data entry frequently.
  • Use vision and depth perception for near and far distances and identify and distinguish colors frequently.
  • Use smell frequently to detect or recognize odors.
  • Use hearing frequently to receive oral communication, distinguish body sounds, and hear alarms or malfunctioning machinery.
  • Lift, carry, push, or pull objects weighing 25–50 pounds occasionally.
  • Lift or move patients occasionally.
  • Bend, stoop, kneel, squat, twist, grip, and perform repetitive foot, leg, hand, and arm movements occasionally.
  • Crawl and run rarely.
