Requirements
  • Graduate of an accredited School of Nursing.
  • Registered Nurse licensed by the state.
  • At least 2 years of nursing experience in home hospice or skilled home care.
  • Ability to market and deal tactfully with customers and the community.
  • Excellent assessment and problem-solving skills.
  • Ability to communicate well verbally and in writing.
  • Ability to demonstrate self-confidence and a positive attitude toward self and others while maintaining commitment and enthusiasm toward goal achievement.
Responsibilities
  • Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel.
  • Provide supervision and guidance to clinical staff to ensure service quality and maintenance of standards.
  • Perform chart audits.
  • Gather data for Quality Assurance and Performance Improvement and participate in related activities.
  • Appraise clinical staff performance during probationary and annual reviews, and provide teaching and counseling to enhance service delivery and the professional development of nursing and health aide staff.
  • Identify and evaluate patient needs and the appropriateness of home care by reviewing the initial intake screening form before assigning direct care staff.
  • Assist with the orientation of new staff.
  • See patients in the home setting as needed.
  • Participate in audits, interdisciplinary group meetings, and Quality Assurance and Performance Improvement activities.
  • Maintain adequate and appropriate medical supplies.
  • Provide orientation for new personnel and in-service training for current staff.
  • Ensure that Medicare, state, and Community Health Accreditation Partner regulations are followed by all clinical staff.
  • Pursue ongoing professional development through formal education, workshops, conferences, professional and related organizational meetings, individual research, and reading.
