Requirements
  • Two years of experience in outreach case management or a related field.
  • Have employment eligibility verification and authorization to work for any employer in the United States.
  • Have or be able and willing to obtain CPR and First Aid training.
  • Successfully complete a tuberculosis test, background screening, and drug test as conditions of hire.
  • Have reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
Responsibilities
  • Conduct daily street outreach to develop relationships and build rapport with clients to assist them with finding a home.
  • Respond within specific contract timeframes to requests for street outreach services and send follow-up information to the requestor and supervisor.
  • Conduct the Vulnerability Index-Service Prioritization Decisions Assistance Tool assessment for all new clients, complete the match initiation form, and submit required paperwork to the Coordinated Entry System coordinator.
  • Collaborate with clients and the street outreach team to develop an Individualized Service Plan addressing short- and long-term goals.
  • Provide street-based case management, including regular client meetings, goal support, documentation assistance, employment assistance, housing and health referrals, benefits assistance, housing navigation, landlord advocacy, referrals, linkages, and advocacy for identified needs.
  • Collaborate with community substance abuse and mental health providers to ensure seamless referral services.
  • Build relationships with prospective landlords and identify appropriate permanent housing options.
  • Work with local law enforcement and interim service providers to support transitions from street living to interim housing.
  • Develop and use community resources to connect clients to services.
  • Provide a high-quality customer service environment for clients.
  • Participate in staff meetings and assigned training.
  • Complete necessary paperwork to track client progress, including daily logs and Individualized Service Plans.
  • Ensure program data is accurate and entered into the appropriate documentation systems, including the Homeless Management Information System, internal spreadsheets, and the Coordinated Entry System.
  • Prepare case-related reports covering demographics, outcomes, successes, and challenges.
  • Generate client data for reporting.
  • Complete follow-up and retention services and provide backup documentation in client files.
