Requirements
  • Bachelor's Degree (BSN) and current New York State License or compact License in good standing (or an associate's degree in nursing with relevant homecare clinical experience)
  • Minimum of 1-2 years of Nursing experience with a concentration in the homecare agency space
  • Experience in geriatric care or working with individuals with disabilities
  • Ability to travel within the five boroughs, as needed
  • Current knowledge of state and federal home health regulations
  • Strong interpersonal and communication skills
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook, etc.)
  • Excellent verbal and written communication, organizational and time management skills
  • Strongly Preferred bilingual in Spanish, Russian, Chinese, Korean and Bengali
  • Ability to work collaboratively with caregivers and families
  • Proficient in electronic health record (EHR) systems
  • Required valid driver’s license and reliable transportation
  • Previous experience in a non-skilled home care setting
  • Strong experience using a Uniform Assessment System (UAS)
Responsibilities
  • Conduct initial assessments of clients to determine care needs and develop individualized care plans
  • Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities
  • Monitor clients’ overall well-being and report any changes to the appropriate healthcare professionals
  • Educate clients and their families on effective care strategies, safety measures, and daily living skills
  • Ensure compliance with agency policies, state regulations, and best practices in non-skilled care
  • Maintain accurate documentation of client care and progress, while ensuring confidentiality
  • Act as a liaison between clients, families, and caregivers to facilitate communication and enhance the quality of care
  • Participate in ongoing training and professional development to stay current with best practices in home care
Desired Qualifications
  • Master's degree in nursing
  • Experience in Home Care, Geriatrics and Case Management; field experience assessing patient care
  • 3+ years of homecare agency experience
