- Completion of a nursing assistant program with a certificate of completion, acting only within the scope of authority to practice.
- At least two years of experience working with seniors in congregate housing, long-term care, or hospital settings.

- Administer medications within training and applicable scope of practice.
- Provide respectful, dignified, and individualized care according to each participant’s care plan.
- Observe and report changes in participant condition, behavior, or functional abilities to the appropriate supervisor.
- Safely assist participants with transfers and mechanical devices according to training and safety protocols.
- Ensure participant safety during transfers, repositioning, and mobility activities.
- Support participants with varying physical, cognitive, and behavioral needs.
- Engage participants in meaningful daily interactions and activities that enhance quality of life.
- Maintain a safe, clean, and supportive living environment.
- Monitor participant well-being and promptly report concerns, incidents, accidents, or emergencies to leadership.
- Assist with meal preparation and dining support.
- Provide snacks and hydration as directed by participant care plans.
- Perform daily housekeeping duties, including cleaning, vacuuming, and mopping participant rooms.
- Sanitize bathrooms and high-touch surfaces.
- Complete participant laundry according to established schedules and standards.
- Change bed linens and maintain cleanliness throughout the community living environment.
- Assist participants with morning routines so they are dressed, groomed, and ready for the day center and transportation when applicable.
- Upon participants’ return from the day center, assist with evening routines, meals, personal care, medication administration when applicable and trained, and bedtime preparation.
- Report to work on time and prepared for the assigned shift.
- Take ownership of assigned responsibilities and participant care tasks.
- Maintain confidentiality and professionalism in all interactions.
- Support teamwork, communication, and continuous improvement.
- Adjust assignments as directed by management or the interdisciplinary team based on participant acuity, care needs, and census.
PACE Southeast Michigan provides comprehensive, in-home care for seniors through the Program of All-Inclusive Care for the Elderly (PACE). It helps eligible older adults stay independent at home by coordinating services via an interdisciplinary team that personalizes care for participants and their caregivers. The team meets daily to proactively communicate and coordinate holistic care, aiming to prevent avoidable emergency visits, hospitalizations, and nursing home placements. Serving Wayne, Macomb, and Oakland Counties, the program combines medical and social support to deliver high-quality, integrated care. Its goal is to keep participants healthy and at home for as long as possible while reducing unnecessary hospital use.
