Job Description:

Clinical Social WorkerAssesses, plans, implements, and evaluates patient and family/caregiver needs with respect to patient’s psychosocial health. Provides individual, family, and/or group therapy, creates and supervises an interdisciplinary rehabilitation plan.

SKILLS AND ABILITIES

• Maintains regular, consistent and punctual attendance at the assigned job location
• Demonstrated ability to effectively communicate, both verbally and in writing.
• Must demonstrate strong interpersonal and organizational skills, to work effectively in a fast-paced environment with rapidly changing priorities and competing demands.
• Knowledge of community resources and how to access
• Able to integrate care from a variety of sources to meet patient’s needs
• Ability to read, write legibly, and converse effectively in the English language
• Must possess a high level of critical thinking, priority-setting, and decision making abilities; able to function independently
• Able to work flexible schedule

EDUCATION/CERTIFICATIONS/LICENSES/DEGREES

• Master's degree from a program accredited by the Council on Social Work Education
• Current Oregon Licensure in good standing as a Licensed Clinical Social Worker or working towards obtaining Oregon Licensure
• American Heart Association Healthcare Provider Basic Life Support (BLS)
• SAFE class or organization approved workplace violence training or obtain within six (6) months of hire which must be obtained by the last day of the month after the six (6) month grace period

EXPERIENCE

• At least 2 years’ experience in field of Social Work, Behavioral Health or related field

• Able to stand, walk, bend, squat, reach, and stretch frequently
• Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs
• Have adequate hearing and visual acuity, including adequate color vision
• Must use standard precautions due to threat of exposure to blood and bodily fluids
• Ability to communicate effectively through reading, writing, and speaking in person or on telephone
• Able to use a computer
