Requirements
  • A strong understanding of substance misuse issues and experience working within a similar field.
  • Experience working with groups or pods and managing group dynamics, including conflict or challenging behaviour.
  • Strong verbal and written communication skills and good information technology skills.
  • The ability to work proactively and independently, with effective time management and prioritisation skills.
  • A good working knowledge of mental health interventions, services, and good practice.
  • The ability to work in partnership with professionals, agencies, and internal and external stakeholders.
  • The ability to manage change in a way that prioritises service users' needs.
  • An understanding of information governance processes and the ability to apply necessary safeguards.
  • A commitment to learning and broadening professional knowledge and skills.
  • The role requires an enhanced Disclosure and Barring Service check.
Responsibilities
  • Engage service users and maintain their engagement with treatment and support.
  • Support young adults transitioning between young people's services and integrated adult services.
  • Assess individual needs and formulate treatment and risk management plans using evidence-based assessment and screening tools linked to substance use.
  • Deliver identification and brief advice, extended brief interventions, and structured psychosocial interventions.
  • Provide harm-reduction advice, information, and interventions.
  • Support young adults in accessing medical and clinical interventions under the oversight of the service healthcare and medical team.
  • Deliver relapse-prevention interventions.
  • Work with families and carers involved in the young adult's care plan.
  • Work with relevant agencies involved in the young adult's care plan.
  • Maintain high professional standards and a service-user-focused approach.
  • Contribute to and oversee the learning and development of others.
