Requirements
- A valid State of Ohio Licensed Practical Nurse license is required.
- A valid driver's license and use of an automobile are required because the role involves travel.
- Annual MUI incident training is required as dictated by the State of Ohio Medicaid, Ohio Home Care Program, and Department of Developmental Disabilities.
- The employee must be able to read and understand agency documents, including Plans of Care, flow charts, client charts, policies, and procedures.
- The employee must be able to use a computer, laptop, or tablet.
- The employee must be able to think independently, recognize potential problems, gather information, and determine when to contact a supervisor.
- The employee must provide customer service and client care within agency and program rules.
Responsibilities
- Administer medications in accordance with standards of practice.
- Encourage social and rehabilitative skills, self-care, and independence.
- Perform client treatments in accordance with the Plan of Care.
- Assist consumers in learning appropriate self-care techniques.
- Prepare clinical and progress notes documenting care provided.
- Monitor and document client conditions and report changes to the Clinical Director or Clinical Supervisor.
- Promote activities that achieve a safe and healthy environment.
- Use aseptic technique when performing client procedures and care.
- Collaborate with clinical team members to develop and improve the Plan of Care.
- Assist clients in achieving established goals in the RN-developed Plan of Care.
- Maintain productivity according to agency standards.
- Maintain the required license, continuing education units, and mandatory employment requirements.
- Participate in client care meetings, in-services, orientation, and staff meetings.
- Provide services in accordance with the agency's philosophy, standards, policies, and scope of practice.
- Participate in Performance Improvement activities and adhere to HIPAA guidelines.
- Perform other duties as assigned.
