Requirements
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum 2 years of experience as a nurse in hospice or home health environment
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Responsibilities
  • Provide skilled end-of-life care to patients and families while serving as a trusted mentor, educator, and clinical resource for nurses throughout their onboarding and professional development.
  • Manage a patient caseload, coordinating individualized care from admission through ongoing visits while building meaningful relationships with patients, families, physicians, and the interdisciplinary care team.
  • Guide new clinicians through orientation, provide ongoing coaching and remedial education when needed, and help students and staff build the confidence and competencies required to deliver outstanding hospice care.
  • Work closely with clinical leadership and the Education Department to promote best practices, reinforce quality standards, and support a culture of continuous learning.
  • Through clinical expertise, mentorship, and compassionate leadership, directly influence patient outcomes and the success of the clinicians who care for them.

A nurse practitioner visits patients at home and develops individualized care plans that cover behavioral health, dementia, geriatric, diabetic, wound, and orthopedic care. This in-home, NP-driven approach offers a broad suite of geriatric-focused programs rather than only post-surgical or rehabilitation services. Its goal is to keep seniors safe and healthy at home by coordinating comprehensive, specialty care in their living environments.
