Requirements
  • An undergraduate degree or diploma in a clinically related field, health informatics, computer science, or equivalent is required.
  • Epic Ambulatory certification is preferred, or certification must be obtained within three months of hire.
  • Two to five years of hospital clinical and/or hospital information systems and electronic medical record experience is required.
  • Experience leading collaborative initiatives involving multiple, diverse stakeholders is required.
  • Strong knowledge of the patient journey, transitions into the community setting, and available patient supports is required.
  • Strong interpersonal, communication, and conflict-management skills are required.
  • Experience supporting large-scale health care information systems and technologies is required.
  • Project management skills are required to execute on-time organizational deliverables.
  • Experience with Microsoft Office, Windows, and Visio is required.
  • A proven attendance record, strong organization, and attention to detail are required.
  • The ability to perform the essential duties of the job and comply with patient and staff safety policies and procedures is required.
Responsibilities
  • Co-design with Western York Region Ontario Health Team and Mackenzie Health leadership and support execution of the digital health primary care strategy.
  • Collaborate with community partners in primary care to analyze care models and workflows and develop tools for community care settings.
  • Develop relationships with community partners to understand referral patterns and identify opportunities to improve visibility into patient records.
  • Expand integrated care pathways for chronic disease management and population health management across the care continuum.
  • Create longitudinal care plans for patients transitioning home to primary care management.
  • Lead collaborative initiatives involving nursing and allied health leadership and diverse clinical groups.
  • Provide end-user support and troubleshoot system issues and errors.
  • Act as the liaison between operational end users, the electronic medical record, and applicable third-party vendors to establish appropriate workflows.
  • Analyze business operations, prioritize system changes, and improve the system while following system controls and electronic medical record best-practice standards.
  • Maintain the system through system build protocols, technical testing, integrated system testing, and migration across multiple environments.
  • Support quarterly system upgrades and optimize software functionality for end users.
  • Translate knowledge among team members to support training curriculum development and delivery.
  • Develop policies and procedures supporting clinical and non-clinical workflows.
  • Participate in rotational on-call support for end users after business hours.
Desired Qualifications
  • A master's degree, such as business administration, health informatics, or health administration.
  • Certification in Epic Ambulatory and Cadence.
  • Experience working directly with primary care practices.
  • ITIL certification.
  • Clinical or professional designations.
  • Certification in Healthy Planet.
  • ServiceNow experience.
