Requirements
  • A current Physical Therapist license in the state of employment is required.
  • At least one year of experience as a Physical Therapist in an acute setting is required.
  • The ability to communicate effectively with patients, families, physicians, and coworkers is required.
  • Knowledge of physical therapy best practices is required.
  • The ability to make appropriate physical therapy judgments is required.
  • The ability to identify situations and handle them with an appropriate solution is required.
  • A detail-oriented and observant work style is required.
  • The ability to prioritize work and complete it in a timely manner is required.
  • A valid driver's license and auto insurance listing the employee as a driver are required.
  • The employee must meet the physical requirements of the position, including walking, standing, bending, stretching, reaching, climbing, stooping, and lifting up to 50 pounds with or without assistance.
  • The employee must provide evidence of a current negative tuberculin skin test or chest X-ray, or receive TB testing at orientation, followed by annual testing unless prior documentation is available.
  • The employee must be able to read, speak, and write English and use a computer or personal digital assistant.
  • The employee must complete required clinical documentation within agency, Medicare, and federal guidelines.
Responsibilities
  • Provide skilled physical therapy in patients' homes as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff.
  • Perform OASIS assessments to develop individualized plans of care and adjust them as patient needs change.
  • Complete initial evaluations and admission visits within 48 to 72 hours of referral.
  • Develop and follow plans of care within the scope of practice for a licensed Physical Therapist.
  • Reduce rehospitalizations by front-loading visits for high-risk patients and teaching patients preventive measures and appropriate decision-making.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, and water.
  • Arrange outpatient services that cannot be provided in the patient's home.
  • Promote continuity of care through appropriate admissions, transfers, and discharges.
  • Counsel patients and families about diseases or injuries and their management throughout all phases of physical therapy.
  • Incorporate patients and families into plan-of-care development.
  • Instruct patients and family members in the proper use of wheelchairs, braces, walkers, crutches, canes, and other prosthetic or orthotic devices.
  • Coordinate needed equipment financial-responsibility information through the durable medical equipment department and/or medical social worker.
  • Listen to patients and family members.
  • Oversee and supervise therapy services provided by physical therapist assistants and, when appropriate, nurse assistants.
  • Evaluate plans of care based on patient response and medical condition, and adjust them as needed.
  • Communicate supervisory-visit results to physical therapist assistants and nurse aides.
  • Complete clinical documentation, including treatment, assessments, patient education, evaluations, recertifications, resumptions of care, and discharges, within the specified timeframes.
  • Complete chart audits at the agency site as requested.
  • Communicate schedule changes to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Participate in orientation and training of new employees.
  • Drive to patients' primary locations for scheduled visits and attend assigned visit locations daily.
  • Maintain an active Physical Therapist license in the state of employment.
  • Follow agency policies and procedures.
