Requirements
  • Graduate of an accredited School of Practical Nursing (Licensed Practical Nurse or Licensed Vocational Nurse).
  • Current appropriate state licensure.
  • Successfully complete a training course in the theory and practice of hemodialysis.
  • Successfully complete cardiopulmonary resuscitation certification.
  • Must complete the Nurses Technical Training Program upon hire or soon thereafter.
  • Good verbal and written communication skills.
Responsibilities
  • Provide direct patient care for assigned patients under the direction of a Registered Nurse and in compliance with physician orders, state nurse practice laws, company policies and procedures, and regulatory requirements.
  • Administer intravenous medication in accordance with physician orders and state nurse practice laws.
  • Assist the Home Therapy Registered Nurse/Coordinator in identifying, evaluating, and training candidates for the home dialysis program.
  • Document patient training and treatment information accurately in the individual patient record and Medical Information System.
  • Monitor, assess, and intervene for home patients through home visits, clinical appointments, additional training sessions or reviews, laboratory studies, and annual studies such as X-rays and electrocardiograms.
  • Respond appropriately to patient problems or emergencies and take the correct actions to achieve resolution.
  • Comply with patient supply procedures, maintain appropriate documentation, and maintain durable medical equipment, including preventive maintenance and water analysis.
  • Perform monthly chart audits and report the results to the Registered Nurse/Coordinator or Manager.
  • Monitor the peritonitis rate and peritoneal equilibration test results and report outcomes to attending physicians, the Quality Assurance Committee, and the Home Therapy Registered Nurse/Coordinator or Manager.
  • Collaborate with physicians and the Home Therapy Registered Nurse/Coordinator or Manager to change patient treatment plans as needed.
  • Collaborate with the Home Therapy Registered Nurse/Coordinator or Manager and physicians to schedule backup treatments when necessary.
  • Educate pre-dialysis and existing end-stage renal disease patients about treatment options.
  • Prepare home supply inventory packages and monitor the efficient use of supplies.
  • Maintain current knowledge of dialysis-related equipment, including hemodialysis machines, water treatment systems, peritoneal cyclers, and peritoneal assist devices.
  • Participate in scheduled in-service classes on current dialysis principles and techniques.
  • Troubleshoot equipment problems with patients by telephone and ensure appropriate resolution.
  • Address blood spills immediately according to bloodborne pathogen control policies.
  • Participate in scheduled staff meetings.
  • Assist with short-term and long-term patient care plan meetings.
  • Assist with educational in-services about home therapies for in-center staff, hospitals, or nursing programs as needed.
  • Participate in training facility staff in the principles, concepts, and practice of the customer service model.
  • Act as a resource person for other staff members.
  • Travel between assigned facilities and various community locations as required.
Desired Qualifications
  • One year of medical-surgical nursing experience is preferred.
  • Hemodialysis experience is preferred but not required.
  • Intensive care unit experience is preferred but not required.
