Responsibilities
  • To represent the Sharp Health Care continuum to the public and health care community improving service linkages, maximizing care continuity and satisfaction.
  • The CCM will develop strong relationships with physicians, their office staff, community clinics and healthcare providers to assure that there are multiple options for patients and are provided excellent care.
  • The Case Manager will be responsible for collecting data regarding patient referrals, quality outcomes and reporting to oversight committees and organizations.
  • The primary purpose of the CCM is to reduce anxiety and frustration with coordinating care and improve patient satisfaction with the health care systems.
  • The Case Manager will facilitate more efficient and cost effective use of health care resources.
  • The Clinical Case Manager's role is to be a patient advocate providing patients and their families with a point of care approach, and guiding them through the healthcare system providing information regarding services, education, funding and access to care.
  • The Clinical Case Manager will be responsible for coordinating care among health care providers, and supporting the patient journey through the health care system.
  • Build Relationships - Identify personnel in departments involved in the care of oncology patient's including: physicians, nurses, imaging, social services, radiation oncology hematology/oncology and obtain cooperative referral sources.
  • Build Relationships - Establish excellent communication to assist with data collection and follow up care.
  • Build Relationships - Facilitate interaction and communication with health care staff and providers.
  • Build Relationships - Represents Sharp HealthCare when in contact and/or collaboration with professional organizations within the community.
  • Community Outreach Activities - Formulate relationships with members of the community by visiting churches, synagogues, schools, libraries to increase cancer awareness and involvement in community programs.

Sharp Rees-Stealy opens 75,000 sq ft medical centre in Chula Vista with 375-space car park

PMB, a healthcare real estate developer, has completed a 75,000-square-foot medical outpatient building for Sharp Rees-Stealy in Chula Vista, California. The three-storey facility consolidates services from two existing sites, offering expanded primary, urgent, specialty and rehabilitation care. Located at Chula Vista Center Mall on H Street, the building features imaging services, laboratory facilities, a pharmacy, and multiple specialty departments including cardiology, neurology and sports medicine. A 127,000-square-foot parking structure with 375 spaces connects to the medical centre via a second-floor pedestrian bridge. The project continues PMB's partnership with Sharp HealthCare dating back to the 1970s. HGW served as architect, with Pacific Building Group as general contractor. PMB has developed over 130 healthcare facilities totalling approximately 6.5 million square feet.
