Requirements
  • A Bachelor's degree in a Human Services field, an Associate's degree in a Human Services field plus at least two years of experience in a Human Services field such as advocacy or case management, a Case Management certification plus at least two years of experience in a Human Services field such as advocacy or case management, or at least eight years of experience in a Human Services field such as advocacy or case management.
  • Ability to work in a fast-paced environment and under pressure.
  • Ability to be flexible and multitask.
  • Ability to work effectively with populations affected by vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, or other conditions affecting housing stability.

  • Ability to self-motivate and organize tasks with service users, often in non-therapeutic environments.

Responsibilities
  • Advocate for the organization's mission and uphold the agency's values.
  • Deliver services with safety, respect, and effectiveness.
  • Follow organizational policies and procedures.
  • Assist participants in accessing community resources and scheduling and attending appointments.
  • Maintain professional rapport with service users, peers, and community service providers.
  • Use assessment information to establish client-centered goals and develop service plans with measurable objectives and completion timeframes.
  • Apply the Housing First philosophy by connecting individuals and families experiencing homelessness to permanent housing without preconditions or barriers to entry.
  • Communicate with internal and external departments regarding case coordination and complete safety plans and risk minimization measures to support clients and residents.
  • Provide referrals to appropriate service providers within and outside the agency.
  • Maintain confidentiality in accordance with agency policies, state regulations, HIPAA, and appropriate client boundaries.
  • Balance caseload needs based on acuity, program duration, and housing stability while supporting 20–30 individuals at one time.
  • Advocate for clients and residents as needed and transport clients to and from appointments when necessary.
  • Review and evaluate resident progress on an ongoing basis and adjust case plans as appropriate.
  • Attend required trainings and participate in voluntary trainings as needed.
  • Maintain administrative duties, including current paper and electronic documentation for program eligibility, compliance, and time and effort.
  • Perform other duties as assigned.

CHERRY HILL, N.J.--(BUSINESS WIRE)--TD Charitable Foundation, the charitable giving arm of TD Bank, America's Most Convenient Bank®, today announced it awarded a total of $7 million to 37 non-profits across the bank's Maine to Florida footprint and in Michigan through the 18th annual Housing for Everyone grant program.
