Requirements
  • One to two years of experience or training in a related function.
  • A high school diploma or equivalent is required.
  • A valid driver's license, adequate insurance when using a personal vehicle, and a safe driving record are required. Applicants must be at least 21 years of age, have maintained a valid United States driver's license for at least six months, and pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, operate a computer and office equipment, sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Ability to assist clients with activities of daily living.
  • Ability to communicate with clients, staff, and external representatives.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Program certification is required within 150 days of hire.
  • Safety Care certification is required within 90 days of hire.
  • Positive Behavioral Supports Tier II certification is required prior to hire.
  • NET certification is required.
  • Knowledge of administrative practices, personal computer applications and equipment, advocacy techniques, empowerment and self-advocacy techniques, family partnerships, social inclusion, participatory planning, physical personal management, household management, and crisis intervention techniques.
  • Ability to work collaboratively and independently, multitask in a time-sensitive environment, organize work, exercise judgment, solve problems and make decisions in stressful situations, adapt communication styles, teach, maintain confidentiality, learn new skills, and communicate effectively orally and in writing.
Responsibilities
  • Assist individuals in becoming fully integrated into their communities and develop and maintain friendships.
  • Assist individuals in accessing community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Empower individuals to make choices, advocate for themselves, and participate in decisions about their supports and lives.
  • Support access to assistive technology and adaptive equipment to augment communication, access communities, and promote independence.
  • Support individuals in securing, maintaining, and growing work, volunteer, and other community engagement opportunities.
  • Support individuals and their families in maintaining strong partnerships with staff and other interested parties.
  • Provide administrative support to the Program Director, including scheduling staff assignments, training new staff, and assisting with clinical paperwork oversight.
  • Function as designated lead staff for shift management and clinical programming.
  • Assist in developing, implementing, and documenting treatment, including individual support plans, person-centered support plans, and clinical programs.
  • Provide training and support involving rehabilitative and behavioral interventions, fire and safety regulations, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning and cooking, household chores, and use of generic resources.
  • Perform hands-on program tasks to implement support plans.
  • Provide transportation as necessary.
  • Assess individuals' physical and emotional status and report changes to a supervisor.
  • Monitor individual funds as needed.
  • Maintain individual records, collect behavioral and academic data, maintain progress notes, and document incidents, programs, and routine activities.
  • Procure, store, administer, document, and dispose of medication in accordance with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Communicate with and provide information to program nurses, case managers, and Visiting Nurses Association representatives.
  • Participate in mandated training and staff meetings.
  • Assist with orienting new staff.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Ensure home safety and maintenance, including housekeeping, snow shoveling, and trash removal.
  • Complete communication logs, community membership logs, activity schedules, and other required paperwork.
  • Provide shift coverage in other homes or programs as assigned.
  • Ensure required staff-to-individual ratios are maintained and assist the Program Director in securing shift coverage.
  • Function as a human rights advocate, provide physical intervention or First Aid during behavioral or medical emergencies, and report to the Disabled Persons Protection Commission.
  • Perform other related duties as required.
