Requirements
  • Must possess, as a minimum, a nursing degree from an accredited college or university, or be a graduate of an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
  • Must be able to read, write, speak, and understand the English language.
  • Must be able to read technical procedures and comprehend policy and procedure manuals.
  • Must be able to effectively present information and respond to questions from managers and employees.
  • Must be able to relate information concerning a resident’s condition.
  • Must apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

  • Must be able to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary to provide quality care.
  • Must possess a current CPR certification.
  • Must maintain all required continuing education and licensing and remain in good standing with the State Board of Nursing.
  • Must meet the facility’s general health requirements, including a medical and physical examination.
  • Must be able to move intermittently throughout the workday and push, pull, move, or carry at least 25 pounds to a minimum height of 5 feet and a minimum distance of 50 feet.
  • May be required to assist in evacuating residents during emergencies.
Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and facility guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Maintain current nursing procedures and participate in quality assurance activities and facility surveys.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Admit, transfer, and discharge residents as required.
  • Complete administrative records, medical forms, reports, evaluations, studies, charting, census reports, charge slips, and accident or incident reports.
  • Protect resident health information and facility information-system credentials and report unauthorized access or suspected violations.
  • Complete admission, transfer, discharge, medication, treatment, and routine resident documentation.
  • Receive and transcribe physician orders and chart nurses’ notes, accidents, incidents, diet changes, and resident care.
  • Prepare and administer medications as ordered, verify resident identity, maintain medication supplies, and ensure accurate narcotic records.
  • Notify the Nurse Supervisor and attending physician of medication discrepancies, automatic stop orders, resident accidents, incidents, or changes in condition.
  • Participate in staffing evaluations, determine shift staffing needs, develop work assignments, provide leadership, and monitor personnel performance against nursing standards.
  • Conduct rounds, receive and give nursing reports, address complaints and grievances, and coordinate nursing services with other departments.
  • Orient residents, families, and new employees to the facility, shift, policies, procedures, and job duties.
  • Provide and coordinate resident care, treatments, rehabilitation, diagnostic services, transportation, dietary requirements, and restorative programs.
  • Perform clinical services including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage-bag care, specimen collection, vital-sign measurement, range-of-motion exercises, and monitoring of seriously ill residents.
  • Review resident charts and care plans, ensure care follows residents’ plans and wishes, and assist with MDS, RAP, and trigger implementation.
  • Implement infection-control, safety, sanitation, emergency preparedness, and hazardous-exposure procedures.
  • Maintain adequate medication, equipment, supply, personal protective equipment, and material safety data sheet inventories.
  • Participate in staff development, orientation, in-service training, continuing education, and license-maintenance programs.
  • Provide data to the Quality Assurance and Assessment Committee and report suspected fraud or false billing.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
