  • A high school diploma or equivalent is required.
  • Experience working with individuals with disabilities or in a similar role is preferred.

  • The candidate must have basic computer skills, including proficiency with Microsoft Office and electronic health records systems.
  • Level 1 requires a high school diploma or GED equivalent and is an entry-level position with a caseload of up to 20 clients.
  • Level 2 requires two years of full-time experience performing advanced journey-level or specialized clerical duties in an office environment and comparable Case Manager experience; Psychology or Sociology case studies or college credits are a bonus but not required.
  • Level 3 requires three years of full-time experience performing advanced journey-level or specialized Case Manager duties in an office environment and comparable Case Manager experience; Psychology or Sociology case studies or college credits are required.
  • The candidate must be able to stand for long periods and perform repetitive tasks.
  • The candidate must be comfortable working in various community settings and environments.
Responsibilities
  • Assist clients in developing and implementing day habilitation plans that reflect their interests, capabilities, and needs.
  • Provide prompts, reminders, guidance, and role modeling in socialization, personal hygiene, exercise, nutrition, and behavior.
  • Coordinate activities with other departments, medical providers, social workers, schools, and outreach agencies to meet client needs.
  • Develop and provide structured services, activities, behavior modification, and social skills as part of rehabilitation skills.
  • Implement wellness and recovery philosophy and values through education and outreach programs to enhance client skills and independence.
  • Encourage clients to participate in community activities and events.
  • Conduct community outreach to engage clients and connect them to resources in settings such as shelters, parks, and community events.
  • Advocate for clients to ensure they receive necessary support and services.
  • Assist clients with applications for public benefits and other resources.
  • Maintain accurate records of client interactions, progress, and services provided.
  • Use electronic health record systems to document contacts and services.
  • Ensure the safety and welfare of clients during activities.
  • Collaborate with staff members, managed care plans, primary care providers, and local programs to coordinate services and support for clients.
  • Facilitate adult workgroups in the Center.
  • Perform outreach services for new clients at Level 2 and Level 3.
  • Work 35 hours per week from Monday through Friday, with occasional evenings and weekends as required by business needs.
Desired Qualifications
