Requirements

Responsibilities
  • Support and promote the mission and values of Covenant Health Ministry.
  • Monitor and manage patient throughput operations, overseeing admissions, discharges, and transfers to ensure efficient bed utilization and care delivery through optimal resource utilization.
  • Collaborate with Hospitalist Medicine and Care Coordination.
  • Provide direct oversight of the Discharge Lounge and Multidisciplinary Rounds.
  • Collaborate with the nursing supervisor and nursing leadership to monitor bed availability and optimize patient placement for overall hospital efficiency.
  • Facilitate code purple events and capacity huddles to optimize patient throughput.
  • Collaborate with the transport team and admission and discharge registered nurses to optimize patient throughput and discharges.
  • Maintain responsibility for bed capacity and management using bed placement and patient throughput tools.
  • Act as an administrative liaison and maintain relationships with external transport agencies, collaborating with the St Joseph Hospital emergency medical services manager.
  • Use evidence and research to develop a strategic plan and implement policies, practices, and workflows that optimize patient throughput.
  • Mitigate barriers to discharge by rounding in departments and attending multidisciplinary rounds to provide assistance and support, with authority to request plan adjustments in the best interest of the patient.
  • Maintain budgets and resources related to patient flow operations and provide monthly updates to the Chief Financial Officer and Chief Nursing Officer.
  • Attend Hospitalist staff meetings and Patient Care Services Executive meetings.
  • Develop and maintain relationships with external partners to facilitate seamless patient transitions.
  • Maintain responsibility for patient throughput metrics and data analysis to identify areas for improvement and implement evidence-based solutions.
  • Coordinate patient flow data to identify trends, forecast bed availability, and prepare reports for leadership decision-making regarding resource allocation.
  • Lead patient throughput initiatives and maintain accountability for operational efficiency and patient experience, including development of key performance indicators.
  • Present data analysis and key performance indicator progress monthly at patient throughput meetings and provide input into the monthly agenda.
  • Act as a resource and mentor for healthcare team members regarding patient throughput, fostering continuous improvement and innovation.
  • Collaborate with departments and stakeholders to identify and address bottlenecks and inefficiencies in patient flow.
  • Collaborate with medical providers as a resource and partner to positively impact patient throughput.
  • Stay current on industry trends and best practices related to patient throughput and develop recommendations for improvement.
  • Ensure compliance with regulations and policies related to patient throughput.
  • Communicate regularly with senior leadership to provide updates and recommendations on patient throughput initiatives.
  • Foster continuous improvement and innovation within the hospital and act as a champion and resource for change.
  • Maintain customer service and patient satisfaction throughout the patient flow process.
  • Train healthcare team members on patient throughput policies and procedures.
  • Communicate and engage with internal and external stakeholders to assess satisfaction and improve patient throughput.
  • Maintain nursing competencies as appropriate to meet or exceed PSC requirements and support the patient care team.
  • Perform other duties consistent with the role.
Desired Qualifications
  • Experience in supervision or bed placement/patient throughput is preferred.
  • A Master of Science in Nursing is preferred.
  • Knowledge of EPIC Electronic Health Record is preferred.
  • Certification in a clinical specialty or nursing leadership is preferred.
