Requirements
  • Graduate of an accredited School of Practical Nursing (LPN or LVN).
  • Hold current appropriate state licensure.
  • Attend and actively participate in all applicable preceptor training programs established by the Regional Education Manager.
  • Attend Fresenius Kidney Care and publicly offered skill-based training as assigned by the Clinical Manager.
  • Maintain a history of high performance, measured by no lower than a commendable performance evaluation rating and no recent corrective actions.
  • At least five years of patient care experience in a hospital setting or related facility is preferred.
  • Meet the necessary requirements of the Ishihara Color Blindness Test as a condition of employment.
  • Successfully complete the Fresenius Kidney Care dialysis training program and CPR certification; continued employment depends on successful completion.
  • Hold intravenous certification if required by the applicable State Nurse Practice Act.
  • Have excellent verbal and written communication skills.
  • Be able to perform frequent, prolonged standing, bending, heavy lifting and patient movement, including occasional assisted movement of machines and equipment up to 200 pounds and lifting chemical and water solutions up to 30 pounds.
  • Be able to work in a healthcare environment with possible exposure to infectious and contagious diseases or materials.
Responsibilities
  • Provide safe and effective hemodialysis therapy under the direct supervision of a registered nurse and in compliance with Fresenius Kidney Care policies, procedures, training, and applicable regulations.
  • Participate in patient care implementation and evaluation and support quality enhancement, quality assurance, patient satisfaction, and process-improvement activities.
  • Participate in recruitment for new clinical staff.
  • Assist with clinical orientation and ongoing training of direct patient care and facility personnel.
  • Serve as a preceptor in direct patient care training programs.
  • Deliver and maintain competency evaluations and audits for other LPNs and patient care technicians.
  • Collaborate with the Clinical Manager and Education Coordinator on quality standards, chronic kidney disease regulatory and legal requirements, and continuous quality improvement.
  • Provide coaching and developmental support to applicable clinical and facility staff.
  • Educate personnel about facility water-system integrity and safety and environmental integrity, patient safety, and staff safety.
  • Maintain communication among Clinical Managers, Education Coordinators, direct patient care staff, and support staff regarding training and organizational-development programs.
  • Implement and potentially design educational programs for direct patient care staff.
  • Monitor patient-care processes through Quality Assurance Indicator activities.
  • Incorporate business policies, procedures, systems, and ethical business practices into training.
  • Educate patients regarding adequacy measures, disease processes, vascular access, end-stage renal disease, dialysis therapy, diet, and medications.
  • Welcome assigned patients and assess their well-being since the last treatment.
  • Evaluate vascular access for patency before and during treatment and perform vascular access cannulation.
  • Obtain and record pre-treatment, post-treatment, and reinfusion vital signs, weights, and dialysis evaluations.
  • Administer heparin as delegated or permitted by state law and administer medications, including intravenous medications, according to physician orders and state nurse-practice laws.
  • Set up and operate hemodialysis machines and initiate dialysis according to prescribed blood-flow and dialysate-flow orders.
  • Evaluate and intervene in intradialytic problems as prescribed or directed and monitor patient response to dialysis therapy.
  • Discontinue dialysis treatment according to established procedures, evaluate patients before discharge and termination of venous access, obtain hemostasis, and apply appropriate dressings.
  • Calculate and enter dialysis-machine programming for ultrafiltration goals, treatment time, sodium modeling, and ultrafiltration modeling.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits 160.
  • Document treatment parameters, vital signs, patient treatment information, progress notes, interventions, and patient responses accurately and timely.
  • Report significant changes in patient condition, unusual incidents, complaints, problems, and unusual findings to the nurse supervisor.
  • Observe patients and conduct machine-safety checks according to facility policy.
  • Obtain prescribed laboratory tests, prepare specimens, and assist with emergency operational procedures.
  • Initiate basic CPR measures during cardiac or pulmonary arrest and respond to dialysis-related emergencies.
  • Conduct infection-control audits, monitor staff and patient adherence, reinforce infection-control policies, and participate in infection-control education and survey responses.
  • Maintain environmental integrity and clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments.
  • Prepare dialysis treatments, perform pretreatment machine-alarm testing including Pressure Holding Testing, and initiate the Solution Delivery System.
  • Clean blood spills with appropriate disinfectant and organize supplies and equipment to prevent wastage.
  • Enter treatment data into the designated clinical application.
  • Review treatment sheets for completeness and nursing signatures and correct omitted entries.
  • Prepare initial patient charts and admission paperwork.
  • Prepare laboratory requisitions and collect, label, prepare, store, and arrange courier collection of laboratory specimens.
  • Travel between assigned facilities and community locations and attend regional, business-unit, or corporate meetings as required.
