- Assist with the pre-admission and admission process and help clients adjust to nursing home placement.
- Interview residents and families to obtain relevant past and present adjustment, behavior, and lifestyle information.
- Document progress and update plans of care at interdisciplinary care conferences.
- Arrange transportation, make appointments, and act as a liaison between departments, residents' families, and outside agencies.
- Assist with handling and accepting placement through one-on-one counseling and group work.
- Ascertain potential and develop discharge plans when clients are admitted.
- Arrange post-discharge services and follow-up care.
- Assess the emotional, financial, spiritual, educational, and social needs of residents and ensure those needs are addressed by the facility or outside agencies.
- Act as a resident and family advocate and ensure residents understand and exercise their rights.
- Organize and conduct Family Council Meetings and maintain meeting minutes.
- Maintain effective communication with residents, families, healthcare professionals, and facility staff.
- Experience in nursing home, hospice, home health, or hospital settings.
