Requirements
  • Graduation from an accredited nursing school and a current state license for Licensed Practical Nurse or Registered Nurse is required at all times.
  • Current CPR or Basic Life Support certification is preferred.
  • Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs such as Outlook, Excel, and Word, web-based platforms, electronic health record systems, and applicable human resources systems.
  • Ability to walk or stand for extended periods and move throughout the community, resident apartments, and other community areas.
  • Ability to stoop, kneel, crouch, crawl, climb, or balance as required.
  • Ability to communicate effectively and identify and interpret written information.
  • Ability to lift up to 50 pounds independently and up to 200 pounds with assistance.
  • Ability to follow infection prevention and control protocols, including use of personal protective equipment when necessary.
  • Ability to complete required health assessments, such as tuberculosis testing or physical evaluations, and pass a background check and drug screening.
  • Ability to use designated personal protective equipment as required by company policy and applicable law.
Responsibilities
  • Provide nursing services in accordance with the nurse practice act and state and federal nursing center regulations.
  • Work with the Director of Nursing to provide direction and oversight to care staff regarding resident needs.
  • Review and revise assessments and care plans at admission, periodically as required by regulation, and when a resident's physical or psychosocial condition changes.
  • Review and transcribe physician orders, provide ordered medical treatments, and assist physicians with resident examinations as needed.
  • Initiate resident education plans according to individualized needs, physician direction, and community policy, including instruction for residents and families.
  • Observe residents, communicate condition changes to physicians, health care proxies, and supervisors, and provide timely interventions as instructed.
  • Administer and document medications according to the five rights: right medication, right route, right resident, right time, and right dose.
  • Supervise and monitor medications for residents who self-administer, when applicable.
  • Document observations, red flags, incidents, interventions, communications, clinical procedures, and daily medication assistance and care throughout the shift.
  • Assist and monitor residents with nutritional and hydration needs and document this care in the nursing record.
  • Assist residents with activities of daily living, including bathing, showering, oral care, toileting, and dressing.
  • Assist residents in scheduling medical, dental, and laboratory appointments.
  • Use proactive interventions, monitoring, and safety checks during room checks and rounds.
  • Answer residents' call lights promptly.
  • Assist residents with transfers and ambulation using proper body mechanics.
  • Measure temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess resident condition.
  • Maintain a restraint-free environment.
  • Support fall-prevention programs for residents who need them.
  • Respond to life-saving situations according to nursing standards, policies, procedures, and protocols.
  • Respond appropriately to personal calls from other health center units in the community.
  • Provide first aid, call emergency personnel, and call family members at residents' request when needed.
  • Review resident service plans for assisted living and memory care.
  • Encourage residents to participate in scheduled facility activities and assist them in getting to activities as needed.
  • Assist with post-mortem care as instructed.
  • Oversee and manage the medication delivery system, including timely order entry, receipt and tracking, daily medication administration record documentation, narcotic counts, and timely medication check-in.
  • Notify the pharmacy and supervisor of medication issues and seek timely resolution.
  • Promote each resident's physical, personal, and emotional well-being and support the resident's maximum level of self-care and independence when possible.
  • Understand residents' advance directives and follow their wishes.
  • Ensure effective communication by community staff with residents' families regarding residents' physical, personal, and emotional well-being.
  • Collaborate with others, take direction from management, and engage in conflict intervention and resolution for residents, families, employees, and others as needed.
  • Participate in staff activities that foster teamwork, unity, and morale.
  • Stay current on required training and ongoing education initiatives and pursue continuing self-improvement.
Desired Qualifications
  • At least one year of nursing experience is preferred, although new graduates are welcome.
