Requirements
  • An MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related primary care specialty.
  • Board certification or board eligibility, with board eligibility required.
  • An active Florida medical license or the ability to obtain one.
  • Experience in primary care, managed care, or value-based care, or completion of an Internal Medicine or Family Medicine residency.
  • Strong clinical judgment and communication skills.

Responsibilities
  • Build lasting relationships with a panel of approximately 500 Medicare Advantage patients.
  • Deliver comprehensive primary care focused on prevention, chronic disease management, and healthy aging.
  • Create personalized care plans that improve quality of life and long-term health outcomes.
  • Partner with medical assistants, care managers, specialists, pharmacists, hospitals, and community providers to deliver seamless care.
  • Lead care plan discussions and ensure smooth transitions across the healthcare continuum.
  • Advocate for patients by removing barriers to timely, coordinated care.
  • Proactively identify and close care gaps to improve quality metrics.
  • Reduce avoidable emergency department visits and hospitalizations through thoughtful outpatient management.
  • Use a value-based approach to improve patient outcomes while lowering total cost of care.
  • Utilize Epic or eClinicalWorks to document care and coordinate with the interdisciplinary team.
  • Use population health data and clinical insights to prioritize outreach and identify opportunities for intervention.
  • Support accurate documentation and coding with the help of dedicated coding specialists.
  • Build trust with patients and caregivers through empathy, education, and meaningful relationships.
  • Empower patients to better understand and manage their health.
  • Contribute ideas that improve clinical workflows, patient experience, and operational excellence.
  • Collaborate with the growing team to help redefine senior primary care.
  • Build and maintain a thriving patient panel of approximately 500 patients.
  • Deliver exceptional patient experiences while maintaining outstanding clinical outcomes.
  • Improve quality measures through proactive, preventive care.
  • Collaborate closely with the care team to continuously improve patient outcomes.
