Requirements
  • Registered Nurse licensed in the Commonwealth of Massachusetts
  • Minimum of 4 years of experience working with adults in a community-based mental health, substance use and/or physical health setting
  • Demonstrated knowledge base and judgment necessary for independent clinical decision making
  • Excellent verbal and written communications skills
  • Strong interpersonal and customer relations skills
  • Ability to use Electronic Health Records (EHR) Systems to document and coordinate services
  • Excellent organizational, time management and problem-solving skills
  • Strong computer knowledge, including proficiency in Windows operating systems and Windows office suites with emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; comfort and experience using mobile technologies
  • Knowledge regarding psychiatric rehabilitation and understanding of recovery model
  • Understanding of Health Equity and competence in serving culturally diverse populations (strongly preferred)
  • Must hold a valid drivers’ license and have access to an operational and insured vehicle and be willing to use it to transport members
  • Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned
Responsibilities
  • Provide medical consultation to MCCN team members regarding acute changes in Enrollees’ status
  • Serve as liaison to and consult with the MCCN Medical Director
  • Conduct medication reconciliations during comprehensive assessments and after inpatient, ED, or 24-hour setting discharges
  • Coordinate with Primary Care Providers to verify discharge medications, address discrepancies, and promote adherence
  • Educate Enrollees on proper medication administration and identify support systems to aid compliance
  • Support Care Coordinators in ensuring access to appropriate medical and specialty care
  • Review and complete medical sections of Enrollee assessments and collaborate on Long-Term Services and Supports components
  • Participate in interdisciplinary team meetings to promote effective communication
  • Assist with staff orientation and training; provide coaching on medical conditions, risk factors, and care team priorities
  • Follow all MCCN policies, protocols, and evidence-based practices while assisting with staff training
  • Apply a person-centered approach in planning and delivering care in coordination with Enrollees and their support systems
  • Address medical risks and emergencies with appropriate interventions; develop medical protocols as needed
  • Complete all documentation and reporting in a timely and compliant manner
  • Maintain strong communication with staff, Enrollees, and care team members to foster collaboration
  • Ensure adherence to HIPAA, agency policies, and performance standards
  • Attend required trainings, staff meetings, and supervision; maintain all certifications and licenses
  • Treat all clients with dignity and respect in alignment with MCCN’s Human Rights policy
  • Perform all duties in accordance with agency standards and expectations

Worcester nonprofits team up to launch $1.1M child care business incubator. April 7, 2026 Seven Hills Foundation and Affiliates, the Guild of St. Agnes, and The Health Foundation of Central Massachusetts have partnered to launch a new $1.1-million incubator space in Worcester designed to help child care providers launch their own businesses. The Family Childcare Success Project incubator is the first of its kind in Central Massachusetts, according to the three Worcester-based nonprofits. The incubator aims to provide resources and training for those interested in launching their own child care businesses while providing on-site care for area families, with a capacity of 20 children. Participants will have up to two years to build their businesses while operating from the space while they develop their client base and work toward establishing a permanent location. The incubator will operate from two classroom spaces at an existing child care center operated by the Guild at 129 Granite St. The project was launched with $1.1 million from The Health Foundation's Synergy Initiative, which provides funding for projects targeting community-identified health issues in Central Massachusetts. Seven Hills Foundation and Affiliates is the largest human services nonprofit in Central Massachusetts, with $466 million in revenue and 1,923 local employees, according to information provided to the WBJ Research Department. The Health Foundation is the seventh largest grant-making foundation and nonprofit in the region, with $88.25 million in assets. The Guild of St. Agnes generates $33 million in annual and has $17 million in assets, according to nonprofit financial tracker Guidestar.
