- A bachelor's degree in social work or a human services field is required.
- Must possess any certifications or licensures required by the state of employment to practice in long-term care.
- Work with patients, residents, and their family members or significant others to identify strengths and social, emotional, and mental health needs and obtain services to meet those needs.
- Provide services in accordance with the National Association of Social Workers Code of Ethics, applicable federal, state, and local guidelines and regulations, company policies and procedures, and standards of care.
- Collaborate with the interdisciplinary team to promote and protect patient and resident rights and well-being, including educating staff on the role of social services.
- Participate in developing a written, interdisciplinary plan of care for each patient or resident to address social-service needs.
- Hold meetings with patients, residents, families, and significant others as requested to provide support and information that improves quality of life.
- Perform patient and resident discharge-planning activities, including family and patient education, nursing-center documentation, and referrals to appropriate agencies and suppliers.
- Establish relationships and maintain contact and referral flow with community-based agencies and services that benefit the patient and resident population.
