Requirements
  • A current New Hampshire Licensed Nursing Assistant license is required.
  • Basic Life Support, cardiopulmonary resuscitation, and automated external defibrillator certification is required.
  • The employee must possess the physical and mental abilities to perform the position's tasks, including sitting, standing, walking, bending, stooping, reaching, and lifting or carrying up to 35 pounds frequently and 50 pounds occasionally.
  • The employee must be able to perform all essential functions with reasonable accommodation if disabled.
  • Crisis Prevention Institute training is preferred or must be obtained before completing orientation.
Responsibilities
  • Provide nursing care to patients within an assigned hospital unit under the direction of medical staff, registered nurses, and licensed practical nurses.
  • Practice according to American Nurses Association care guidelines adopted by the Nursing Division and adhere to the New Hampshire Nurse Practice Act.
  • Perform patient-care activities and related services addressing patients' personal needs and comfort.
  • Document patient care in the Electronic Health Record, including vital signs, intake, output, mobility, pain, skin abnormalities, and relevant patient or visitor interactions.
  • Report unusual findings or changes in patient condition to the primary or charge registered nurse.
  • Complete bedside shift hand-off reports independently and routinely.
  • Answer call lights promptly and use initiatives such as the No Pass Zone to improve response times.
  • Perform bladder scans, electrocardiograms, continuous passive motion procedures, cold therapy assistance, and other appropriate medical procedures after completing required competencies.
  • Bathe patients daily and assist with personal hygiene.
  • Encourage and assist patients with routine ambulation and mobility unless otherwise ordered, and document the activity in the Electronic Health Record.
  • Serve and collect food trays, assist patients who need help with intake, and provide between-meal nourishment.
  • Collect urine and fecal occult samples for testing.
  • Assist with specialty beds, mattresses, skin care, and wound care under a nurse's direction.
  • Provide a secure and safe environment for behavioral health patients through one-to-one observation, constant observation, and continuous visual remote monitoring after completing the relevant competencies.
  • Perform continuous visual monitoring initiated by the primary registered nurse after completing assigned equipment and role competencies.
  • Explain nursing care to involve patients in their care.
  • Rotate among shifts or departments as needed to meet patient-care staffing needs.
  • Assist with Hospital Unit Coordinator duties as necessary.
  • Prepare equipment and assist nursing staff during patient treatments and examinations.
  • Use equipment, safety protocols, and policies when providing patient care and transport.
  • Apply fall-prevention measures for patients assessed as high fall risk, communicate concerns to the registered nurse, and report them to the oncoming Licensed Nursing Assistant.
  • Use AIDET when interacting with patients and assist with hourly rounding.
  • Complete assigned annual competencies by their due dates.
  • Maintain adequate patient-care hours for licensure and current required licenses and certifications.
  • Avoid unauthorized overtime and manage Earned Time Off according to policy.
  • Perform other duties consistent with the role.
