Requirements
  • Tertiary education in a health-related discipline.
  • Current unrestricted General Registration with the Australian Health Practitioner Regulation Agency or relevant professional association.
  • At least one year of aged care experience, including client assessments and/or service delivery.
  • A current Australian driver's licence, a comprehensively insured vehicle, and the ability to travel within the service area.
  • Proficiency in Microsoft Word, Excel, Outlook, and client management systems.
Responsibilities
  • Conduct thorough aged care assessments to identify the needs of older adults.
  • Facilitate referrals to essential services that support independent living.
  • Promote the retention and maximization of function and capability in daily living.
  • Work closely with families and carers as key stakeholders.
  • Ensure assessments comply with Department of Health contractual requirements.
