Requirements
  • Active and valid Massachusetts Registered Nurse license.
  • Active American Heart Association Basic Life Support certification.
  • Associate Degree in Nursing from an accredited school of nursing.
  • Minimum of one year of direct care nursing experience.
  • Compliance with mandatory influenza vaccination as a condition of employment.
Responsibilities
  • Plan, organize, and direct home care services to meet the needs of patients and families within their homes and communities.
  • Complete and document an initial assessment of patient and family to determine home care needs.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Administer medications and treatments as prescribed by the physician.
  • Instruct, supervise, and evaluate the home health aide care provided every two weeks.
  • Identify discharge planning needs as part of the care plan development.
  • Practice confidentiality principles set by the agency and Federal Health Insurance Portability and Accountability Act guidelines.
  • Demonstrate proficiency in Outcome and Assessment Information Set completion.
  • Complete documentation through EPIC in accordance with agency timeliness policies.
Desired Qualifications
  • Bachelor of Science in Nursing from an accredited school of nursing.
  • Experience in the area of public health or home care nursing.
  • Experience with Outcome and Assessment Information Set (OASIS).
  • Experience with EPIC electronic health record system.
