Requirements
  • Nurse Specialized in Wound Ostomy & Continence (NSWOC).
  • International Interprofessional Wound Care Course (IIWCC) may be considered.
  • Canadian Nurses Association Certification - WOCC(C) – an asset
  • Minimum of 2 years’ experience as a practicing Registered Nurse ideally in a community setting and/or providing wound/ostomy/continence care to patients (direct care/consultation)
  • Current membership/registration with the Nurses Specialized in Wound Ostomy & Continence Canada.
  • Knowledge of wound/ostomy and continence standards and clinical best practices.
  • Involvement with demonstrated clinical leadership and teamwork.
  • Competent to assume an educator role.
  • Demonstrated competence and excellence in wound/ostomy and continence assessment, care and management.
  • Demonstrates ability to work autonomously utilizing critical thinking skills/judgment.
  • Registration/licensing with provincial regulatory college in good standing.
  • Working knowledge of applicable legislation including Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act and others.
  • Superior oral and written communication skills to effectively interact with patients and families to ensure barriers to communication do not impact the patient’s ability to direct their own care process.
  • Effective skills in critical thinking and problem solving.
  • Knowledge of applicable community resources available for patients and families.
  • Current certification (within the past 12 mos.) of First Aid, CPR, completed within Canada.
  • A clear Vulnerable Position Screening (VPS) as per policy is required
Responsibilities
  • Provide direct practice to patients requiring wound, ostomy and continence assessment, care and management.
  • Provide education to ParaMed’s nursing health care team, patient and families.
  • Provide consultation /clinical support to ParaMed’s nursing health care team (direct/or remote) including recommendations for specialty products/nursing care.
  • Participate in wound/ostomy/continence clinical leadership opportunities.
  • Collaboration with internal and external stakeholders.
  • Preceptor ParaMed nurses as required.
  • Follow wound, ostomy and continence evidence-based clinical best practices and ParaMed policies and procedures.
  • Obtains informed consent prior to and throughout service provision
  • Works collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resources
  • Provides safe, competent, compassionate and ethical nursing care to ensure the personal safety of patients
  • Uses appropriate assessment tools and techniques in consultation with patients and the health care team to assess patients regarding physical, emotional, spiritual, cultural, cognitive, developmental, environmental and social needs
  • Engages patient using a collaborative approach, building rapport and trust, achieving a mutual understanding and agreement to identify actual and potential patient health care needs, strengths, capacities and goals.
  • Establishes and maintains appropriate professional boundaries with patients and the health care team, including the distinction between social interaction and therapeutic relationships
  • Participates in necessary care conference meetings with ParaMed and Funder.
  • Maintains the knowledge, skills and attitudes to provide safe, efficient, effective care in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice
  • Maintains professional patient records related to patient encounters and plans in compliance with ParaMed’s policies and the standards applied by the regulatory body or Nursing college.
  • Shows respect for cultural needs, dignity, privacy and confidentiality of patients
  • Discharges patient when goals have been achieved
  • On-call may be required based on Branch Operations
