Requirements
  • A Bachelor of Arts or Bachelor of Science degree in Pharmacy is required; a Master of Science or Doctor of Pharmacy is preferred.
  • A current California Pharmacy License is required.
  • Current Basic Life Support/Healthcare Provider status per the American Heart Association is required.
  • Experience in a hospital or healthcare setting is preferred.
  • Must have motor skills to prepare, mix, and transfer drugs aseptically and without spillage, especially chemotherapy.
  • Ability to perform data entry.
  • Must demonstrate the knowledge and skills necessary to provide age-appropriate care to patients served on the unit.
Responsibilities
  • Work under the direct supervision of the Director and Clinical Pharmacy Coordinator to assist in optimizing patients' drug regimens.
  • Maintain appropriate records for medications and pharmaceutical supplies ordered by physicians, dentists, and other qualified prescribers.
  • Perform dispensing and other assigned pharmacy duties in accordance with applicable policies, regulations, and accepted standards of practice.
  • Perform drug regimen reviews to identify therapeutic duplications; review drugs and allergies; assess drug-drug, drug-nutrient, and drug-laboratory test interactions; evaluate dose appropriateness based on age, renal function, and liver function.
  • Assist with patient and family education when necessary.
  • Monitor patients on clinical drug protocols.
  • Attend codes, rounds, and patient care conferences as needed.
  • Assist with pain management as needed.
  • Provide drug in-services to nursing, pharmacy, medical staff, and other ancillary personnel as needed.
  • Identify potential or actual adverse drug events and report them as necessary.
  • Perform other assigned duties.
