- A New York State Registered Nurse license is required.
- At least one year of recent hands-on experience as a Registered Nurse is required.

- Case-manage patients and assess, plan, develop, and implement comprehensive home health care plans.
- Measure vital signs, administer intravenous therapies, change dressings, and clean wounds as needed.
- Provide skilled nursing care, preventive and rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home settings.
- Collaborate with physicians, therapy and social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
- Provide patient and family education on disease management and health promotion as directed by the physician’s plan of treatment.
