Responsibilities
  • Provide one on one assistance to persons with special needs with tailored life plans/goals to enhance developmental skills needed for independent living within their chosen community.
  • Display a congenial, warm acceptance for all individuals as the Community Habilitation Specialist.
  • Be primarily responsible for the assessment and observation of the person’s skills development; be alert to expressions of attitudes, feelings and relationships of each individual; develop awareness of individual needs through observation, case history study and consultation with staff.
  • Support the person in navigating the program and identifying incentives in learning new skills and behaviors.
  • Engage primary caregivers and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
  • Implement problem conceptualization, treatment planning, intervention implementation, and outcome review and strategy revision using the model's Analytic process (Brief Family Systems Therapy and Parental Management Training).
  • Develop Valued Outcomes with steps on how to achieve them based on caregivers/community supports and observed behaviors and child/adolescent strengths.
  • Meet with the assigned children/adolescents 1-2 times per week depending on the child/adolescent coordinated hours.
  • Travel to borough assigned.
  • Attend supervision and training as required; protect the individual’s rights.
  • Document each training session in the appropriate databases and report progress and barriers with each client.
  • Receive consultation and supervision from the Community Habilitation Supervisor.
  • Maintain a positive and strength-focused approach to cases.
