Requirements
  • A current physical therapist license is required.

Responsibilities
  • Perform ongoing clinical assessments, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and assess patient appropriateness for home care.
  • Develop and evaluate the plan of care in partnership with the patient, representative when applicable, and caregivers.
  • Document, prepare, and complete clinical notes, phone orders, and other clinical record documentation on an ongoing and timely basis, and revise the plan of care based on evaluation and ongoing assessment data.
  • Provide physician-ordered services indicated in the plan of care and oversee other team members, when applicable, in patient care consistent with patient needs.
  • Apply infection-control concepts and standard precautions during patient care activities to prevent contamination and disease transmission.
  • Accept clinical assignments consistent with education and competence to care for patients.
  • Provide supervision and guidance to physical therapy assistants and home health aides as appropriate.
  • Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
  • Communicate with physicians and other healthcare practitioners involved in the plan of care regarding the current plan of care.
  • Meet mandatory continuing education requirements of the agency and licensing board.
  • Assist and instruct patients and families in transfer techniques appropriate to patient abilities.
  • Instruct patients on the safe use of walkers, wheelchairs, canes, and other equipment.
  • Use therapy equipment appropriate to patients' diagnoses and needs.
  • Provide patient, caregiver, and family counseling and education.
Desired Qualifications
  • At least one year of home health experience is preferred.
  • Experience in a related healthcare setting is preferred.
  • Knowledge or experience using Homecare Homebase electronic medical record software is a plus.
  • Excellent communication and interpersonal abilities are desirable.
