Requirements
  • High School diploma or equivalent
  • Current unrestricted Paramedic state license
  • A minimum of 5-10 years’ work experience as a Paramedic in Emergency Medical Services (EMS) and/or Mobile Integrated Health (MIH) OR Associate's degree with a minimum of 5 years of work experience as a Paramedic in EMS and/or Mobile Integrated Health (MIH) required
  • Current, valid Driver’s License and reliable means of personal transportation to be used for transporting self, as well as company provided supplies and equipment during home visits
  • Ability and willingness to travel locally and regionally up to 100% of the time
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required
  • Advanced Life Support (ALS) certification required
  • Community Paramedic Certification (CP-C) required within 18 months of hire date
  • Must live and/or work in the Richmond VA area
  • Experience in telemedicine or virtual clinical care delivery strongly preferred to have been gained in prior roles (desirable)
  • Experience working with geriatric patients is a plus (desirable)
  • Spoken and written fluency in English; bilingual Spanish a plus (desirable)
  • Knowledge of HIPAA regulations and compliance standards (Requirements)

Responsibilities
  • With provider guidance and direction, conducts one-on-one advanced, in-home health assessments with senior patients. This includes documenting vital signs and measuring relevant physical attributes as well as identifying cognitive, functional, medication and social challenges impacting patient’s health.
  • Communicates health updates with the provider and virtual care team. Facilitates care plan execution and design to keep patient home, happy, and healthy, when able.
  • Prepares patients for virtual examinations; performs routine screenings, including vitals, weight, EKG, etc. Assists providers with exams and explains treatment procedures and providers' instructions to patients, including facilitating video visits.
  • Prepares and administers medications including IM and IV, changes dressings, applies bandages, removes sutures and other first aid procedures. Performs CPR when necessary.
  • Performs non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic Point of Care lab tests; performs home safety checks to include smoke and carbon monoxide detector operation.
  • Interacts with EMS and emergency department personnel as necessary and with expediency, to coordinate emergency care for any presentation warranting an immediate trip to the hospital or advanced life support services.
  • Creates and records detailed clinical documents for each patient, noting all medications, allergies, and medical conditions presenting. Documents contact information for each family member or friend serving as emergency contact.
  • Documents and reports findings/actions to ensure encounters are accurately recorded and received by the provider as soon as administratively possible. Provides recommendations to provider within scope of license to address/reduce health risk.
  • Maintains home visit supplies, equipment and stocks. Sterilizes instruments, practices OSHA safety standards, practices accurate, legal and ethical documentation at all times.
  • As defined by state statutes and pursuant to medical protocols, policies and procedures set by the provider, administers appropriate patient care at the Paramedic skill level in emergency medical situations.
  • Reports cases of neglect or abuse to Adult/Child protective services. Contacts law enforcement when appropriate.
  • Educates seniors on aging and disability resources and other applicable social services. Recommends to provider and care team needed mental health interventions, leveraging community resources whenever possible. Coordinates fire rescue units, police and other community agencies making home visits.
  • Performs other duties as assigned and modified at manager’s discretion.

Costco Teams With Sesame To Offer Shoppers Health Visits

Costco is offering members medical care via a partnership with online health marketplace Sesame.The deal comes as several retailers expand their healthcare offerings.According to the Sesame website, Costco members can access Sesame’s “best pricing” on online primary care visits ($29), checkups ($72) and mental health visits ($79).Sesame’s goal is “to provide great healthcare to the value-conscious,” said CEO David Goldhill, Bloomberg reported Monday (Sept. 25).Neither Costco nor Sesame has replied to PYMNTS’ request for comment.Goldhill founded Sesame in 2018 out of frustration with a healthcare system he found weighed down by outdated treatment and payment methods. He launched the company as a direct-to-consumer (D2C) online marketplace in 2020. A new membership program followed in 2021.“When I think about how you better control not just the cost of care but also the quality of care and the ability of customers to access it, it’s all about allowing innovation to occur because there are lots of doctors out there who would like to practice medicine in a different way than what the straight-jacket insurance requires,” Goldhill told PYMNTS’ Karen Webster after the membership program debuted.Costco’s partnership with the company is happening amid a boom in retail healthcare efforts, with companies like Walmart and Amazon investing heavily in their healthcare projects.For Walmart, that means the expansion of its physical Walmart Health centers.
