Requirements
  • A Bachelor's Degree in Social Work, Psychology, Education, or a related field is required, along with at least five years of experience in a health care or educational setting, preferably K-12.
  • Proficiency in electronic systems including Google Suite, Trayt, Slack, Athena or Epic, and Calendly.
  • Knowledge of managed care, medical terminology, patient referral processes, claims, and Current Procedural Terminology coding.
  • Experience working with pediatric behavioral and mental health patient populations.
  • Knowledge of educational practices supporting mental health and social emotional learning in K-12 settings.
  • Experience developing, refining, and evaluating patient care plans or student education plans.
  • Bilingual English/Spanish language proficiency is preferred.
Responsibilities
  • Connect patients and families to appropriate mental health and community resources for long-term care and provide ongoing support to ensure effective transitions.
  • Coordinate with professionals, clinical support staff, schools, and families to reduce barriers and improve satisfaction.
  • Maintain confidentiality in all aspects of care and documentation.
  • Collaborate with pediatric psychiatrists, psychologists, licensed counselors, and operational staff to assess and address long-term psychological needs.
  • Assemble clinical background and referral needs for individual patients and document interactions in accordance with professional ethics and regulatory standards.
  • Serve as the lead care coordinator for internal clinical and consultation tracks.
  • Collaborate with supervisors and colleagues to develop and implement program development, project implementation, and process improvements, including internal referral processes and identification of service gaps for specialized populations.
  • Provide oversight and monitoring of projects and initiatives, assign work, and monitor capacity and completion.
  • Work with the supervisor to create and implement onboarding training for new Care Coordinators and other employees.
  • Hire, train, and supervise graduate assistants, interns, or student learners as required by program leadership.
  • Provide guidance in managing and educating on direct service referral pathways to support and advance program growth.
  • Support clinical staff in using internal resources.
  • Lead and manage community partnerships and referrals with identified agencies to ensure ongoing functioning of pathways.
  • Collect and analyze data for assigned programs or pathways.
  • Prepare monthly reports, in collaboration with the leadership team, for submission to the Texas Child Mental Health Care Consortium.
  • Ensure clinical documentation is available for reporting by monitoring compliance within secure data collection systems such as REDCap, Trayt, Athena, or other systems.
  • Update and support electronic medical record integration, including notes and orders.
  • Implement documentation and technology-related special projects.
  • Serve as a liaison for data and data-related projects as assigned.
  • Attend professional development trainings and behavioral health conferences.
  • Participate in institutional committees and workgroups.
  • Assist with grant writing and funding proposals.
  • Support crisis response and emergency referrals as needed.
  • Contribute to policy development and strategic planning.
