Requirements
  • A Bachelor of Science in Nursing is required.
  • Experience in healthcare discharge planning, utilization review, or case management is required.
  • Experience with elective orthopedic surgical patients is required.
  • An active Registered Nurse license from the California Board of Nursing is required.
  • Basic Life Support certification from the American Heart Association is required.
  • Certification as a Certified Case Manager through the Commission for Case Manager Certification or as an Accredited Case Manager through the American Case Management Association is required.
  • Excellent patient clinical review and reporting skills are required.

  • Knowledge of Medicare and commercial contracts for acute and post-acute care is required.
  • Proficiency in working across various information technology systems is required.
Responsibilities
  • Provide patient service coordination for the surgical service line across the pre-operative, hospital admission, and post-acute phases of care.
  • Coordinate perioperative services with patients, families, physicians, the clinical team, and payers as needed.
  • Comply with Enhanced Recovery After Surgery and other alternative payment model requirements for each surgical event when applicable.
  • Identify collaboration opportunities in surgical services to improve patient experience and reduce length of stay and cost.
Desired Qualifications
  • Experience in healthcare home care is preferred.
  • Knowledge of Centers for Medicare & Medicaid Services and Comprehensive Care for Joint Replacement regulations is preferred.
  • A Master of Science in Nursing is preferred.

Stanford Health Care, John Muir Health announce layoffs. Posted: Sep 1, 2026 / 07:45 PM PDT Updated: Sep 1, 2026 / 07:45 PM PDT (KRON) - Stanford Health Care and John Muir Health have announced layoffs, according to the latest WARN report. According to a notice dated Aug. 28, Stanford Health Care will permanently lay off 79 employees at its 1830 Embarcadero Road location in Palo Alto and 16 employees at its 1840 Embarcadero Road location. The layoffs are effective Nov. 2. According to a notice dated Aug. 27, John Muir Health will cut a total of 78 positions at various locations in Contra Costa County.

Move More: Aim for 7,000 Steps Per Day Research has shown that individuals who average at least 7,000 steps per day significantly reduce their risk of cardiovascular death. Participants are encouraged to use their smartphones, smartwatches, or fitness trackers to monitor activity and build movement into their daily routines. 3. Practice Portion Control One of the simplest strategies for weight management is using a smaller plate. Reducing portion sizes can lower calorie intake, support gradual weight loss, and improve risk factors such as high blood pressure, diabetes, and elevated cholesterol. Choosing more vegetables, lean proteins, and minimally processed foods can further improve heart health. 4. Reduce Waist Size by One Pant Size Rather than focusing solely on the number on the scale, the challenge encourages men to reduce their waist size by one pant size before the end of the year. Men with a waist circumference greater than 40 inches face a significantly increased risk of cardiovascular disease.
