Requirements
  • A current and active California Licensed Vocational Nurse license in good standing is required.
  • Current CPR/BLS certification is required.
  • At least two years of nursing experience in a skilled nursing facility, long-term care, rehabilitation, or treatment nursing is required.
  • Knowledge of skilled nursing regulations, infection prevention standards, resident rights, and documentation requirements is required.
  • The candidate must be able to assess resident needs, recognize changes in condition, exercise sound clinical judgment, and escalate concerns appropriately.
  • The candidate must have organizational, time-management, communication, and interpersonal abilities.
  • The candidate must be able to work independently during weekend operations while collaborating with the nursing team and on-call leadership.
  • The candidate must be able to stand, walk, bend, reach, and perform resident-care activities throughout the shift.
  • The candidate must be able to safely assist with resident positioning, transfers, and mobility in accordance with resident needs and facility safe-lifting practices.
  • The candidate must be able to lift, carry, push, and pull supplies or equipment as needed, with or without reasonable accommodation.
  • The candidate must be able to work around infectious diseases, bodily fluids, and other healthcare-related hazards while using appropriate protective equipment and safety procedures.
Responsibilities
  • Perform and document ordered treatments in accordance with physician orders, care plans, facility policies, and applicable state and federal regulations.
  • Provide wound care and skin treatments, including dressing changes, pressure-injury prevention interventions, monitoring of skin integrity, and related documentation.
  • Complete weekly skin assessments and focused skin assessments when assigned or when a change in condition is identified.
  • Measure, photograph when appropriate and authorized, monitor, and document wounds, skin concerns, treatment progress, and resident response to treatment.
  • Notify the Charge Nurse, RN Supervisor, Director of Nursing, physician, responsible party, and other appropriate team members of significant changes in wound status, skin integrity, pain level, infection concerns, or overall condition.
  • Review treatment administration records, physician orders, wound-care recommendations, and care plans to ensure treatments are completed accurately and on time.
  • Assist with implementing treatment orders received from physicians, wound-care specialists, therapy providers, and other authorized clinical professionals.
  • Monitor treatment supplies and communicate supply needs to the appropriate supervisor.
  • Participate in infection-prevention practices, including personal protective equipment use, hand hygiene, isolation precautions, and reporting infection-control concerns.
  • Collaborate with nursing staff to reinforce treatment schedules, skin-integrity interventions, repositioning needs, and resident-specific care requirements.
  • Provide clinical guidance and education to Certified Nursing Assistants and other care staff within the Licensed Vocational Nurse scope of practice regarding skin care, pressure-injury prevention, and treatment-related resident needs.
  • Support admissions, readmissions, incident follow-up, and other nursing duties within the scope of licensure.
  • Maintain complete, timely, accurate, and legible clinical documentation in the electronic health record.
  • Report suspected abuse, neglect, exploitation, injuries of unknown origin, and other reportable concerns in accordance with facility policy and regulatory requirements.
  • Maintain resident confidentiality and comply with HIPAA, facility policies, and applicable privacy requirements.
  • Attend required in-services, staff meetings, and training programs.
  • Perform other related duties assigned by the Director of Nursing, Assistant Director of Nursing, or designee.
