Requirements
  • A Registered Nurse license must be obtained before the hire date or job transfer date.
  • A diploma from an accredited school or college of nursing is required, or the required professional licensure must be held at the time of hire.
Responsibilities
  • Create personalized plans of care from admission to discharge that incorporate patient and family preferences and remove barriers to smooth transitions across treatment.
  • Serve as a bridge between patients, multidisciplinary teams, and post-acute care providers, connecting families with resources to support long-term recovery.
  • Conduct ongoing clinical assessments to ensure patients receive the appropriate level of care while optimizing health plan benefits and clinical resource utilization.
  • Maintain continuous communication with physicians, care transition teams, and payers to align treatment plans and support efficient patient outcomes.
Desired Qualifications
  • A Certified Case Manager credential from the Commission for Case Manager Certification is preferred; ACM is also accepted.
  • Three or more years of recent acute care nursing experience.
  • Two or more years of hospital case management, utilization management, or care coordination experience.
  • Current ACM-RN or CCM certification is preferred.
