Care Manager RN - NICU Weill Cornell, Cohen Hospital Days
The 60-bed private, single-family NICU rooms at New York-Presbyterian Alexandra Cohen Hospital for Women & Newborns are staffed by expert teams. We are seeking a Care Manager to join our dedicated team, someone who is passionate about providing high-quality transitional care across diverse patient populations.
In this role the NICU Care Manager will provide comprehensive case management support for our patients and families and will actively collaborate and consult with treatment teams and external agencies. This role involves discharge planning for patients transitioning onto the next level of care and ensuring continuity of care with optimal patient outcomes. Care Managers also promote a collaborative relationship with the interdisciplinary team to accomplish case management objectives to reduce length of stay, prevent readmissions, provide quality patient care, and anticipate outpatient service needs. We’re seeking a change-agent—someone with the drive, tenacity and leadership necessary to advance patient care at one of the world’s most respected university medical centers.
Other Key Responsibilities include:
Coordinate needed home care services, including nursing, physical therapy (PT), occupational therapy (OT), feeding therapy, and other infusion services
Assess patient needs and ensure appropriate resources are in place to support a safe and efficient discharge plan
Communicate with patients, families, and caregivers to provide education on community resources and address concerns related to post-discharge care
Serve as a liaison between the hospital, home care agencies, and other community resources
Maintain accurate and timely documentation of discharge plans and need follow-up care
This is a day shift, Monday - Friday position with rotating weekend/holiday coverage.
Preferred Criteria
2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
1 year of leadership experience in a healthcare setting
Master’s degree in healthcare or related field
Required Criteria
Bachelor’s Degree in Nursing (BSN)
Current NYS Registered Nurse (RN) licensure
3 to 5 years of health care experience in an acute care setting
Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical

Salary Range:
$122,300-$168,000/Annual
