- Current registered nurse license in good standing with the state nursing board.
- Strong clinical and assessment skills.
- Ability to communicate and interact effectively with patients, families, team members, and other healthcare professionals.
- Ability to work independently and manage time effectively.
- Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs.
- Apply admission and recertification criteria to establish appropriate hospice care levels and patient eligibility.
- Implement, develop, and document plans of care, recommending revisions as necessary to ensure quality and continuity of care.
- Consult with and educate patients and families about disease processes, self-care techniques, end-of-life care, and nutrition and dietary needs.
- Prioritize patient and family needs with members of the interdisciplinary group.
- Determine the scope and frequency of services based on acuity and patient and family needs.
- Assess caregivers’ ability to meet patients’ immediate needs upon admission and throughout care.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Apply infection-control concepts and standard precautions while coordinating and performing patient-care activities.
- Provide clinical direction to hospice aides and licensed practical or vocational nurses to ensure quality and continuity of services.
- Prepare clinical and progress notes demonstrating progress toward established goals and ensure appropriate documentation of patient care.
- Inform physicians and other personnel of changes in patients’ needs and intervention outcomes.
- Evaluate patient and family responses to care.
- Monitor assigned cases for compliance with third-party payer requirements.
- Provide training to other staff as needed.
- Maintain professional growth and competency.
- Promote the agency’s philosophy and administrative policies.
- Knowledge of or experience using Homecare Homebase electronic medical record software.
- Experience in hospice or a related healthcare setting.
