Requirements
  • Current registered nurse license in good standing with the state nursing board.
  • Strong clinical and assessment skills.
  • Ability to communicate and interact effectively with patients, families, team members, and other healthcare professionals.
  • Ability to work independently and manage time effectively.
Responsibilities
  • Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs.
  • Apply admission and recertification criteria to establish appropriate hospice care levels and patient eligibility.
  • Implement, develop, and document plans of care, recommending revisions as necessary to ensure quality and continuity of care.
  • Consult with and educate patients and families about disease processes, self-care techniques, end-of-life care, and nutrition and dietary needs.
  • Prioritize patient and family needs with members of the interdisciplinary group.
  • Determine the scope and frequency of services based on acuity and patient and family needs.
  • Assess caregivers’ ability to meet patients’ immediate needs upon admission and throughout care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Apply infection-control concepts and standard precautions while coordinating and performing patient-care activities.
  • Provide clinical direction to hospice aides and licensed practical or vocational nurses to ensure quality and continuity of services.
  • Prepare clinical and progress notes demonstrating progress toward established goals and ensure appropriate documentation of patient care.
  • Inform physicians and other personnel of changes in patients’ needs and intervention outcomes.
  • Evaluate patient and family responses to care.
  • Monitor assigned cases for compliance with third-party payer requirements.
  • Provide training to other staff as needed.
  • Maintain professional growth and competency.
  • Promote the agency’s philosophy and administrative policies.
Desired Qualifications
  • Knowledge of or experience using Homecare Homebase electronic medical record software.
  • Experience in hospice or a related healthcare setting.
