- State of New York Registered Nurse licensure.

- Ability to send and receive oral and written messages clearly, positively, promptly, and appropriately while maintaining customer confidentiality.
- Provide symptom and medication assessment, evaluation, education, and management to individuals with persistent mental illness.
- Consult with psychiatrists, psychiatric nurse practitioners, hospitals, community providers, clients, and clients’ families or guardians regarding care.
- Administer intramuscular and supply medications as directed by a psychiatrist or nurse practitioner.
- Assess participants’ physical health needs and make appropriate referrals to community-based medical care.
- Ensure pertinent medication-related information is conveyed to hospital staff when a client is hospitalized.
- Provide physical-health education to ACT consumers through individual services and groups as needed.
- Train program staff on diseases, treatment options, and advances in disease management.
- Identify and include clients’ support networks and involved community partners in care coordination.
- Help clients identify and access resources and collaborate with community resources on their behalf.
- Help clients develop healthy coping mechanisms in individual or group settings.
- Conduct baseline and biannual physical-health assessments for all ACT consumers.
- Manage the medication room, MAR book, and other medical documentation.
- Maintain confidential client records and required documentation, including progress notes, in accordance with agency policies and procedures.
- Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed.
- Facilitate access to inpatient facilities for behavioral-health care when needed.
- Provide 80 percent of treatment contacts in the community, including leading groups when clinically indicated.
- Complete documentation in a timely manner and maintain compliance with agency and Office of Mental Health regulations.
- Coordinate services with other ACT team specialists, including the psychiatrist, substance-use specialist, employment specialist, and nurse.
- Represent the agency and its services to the community in a positive and professional manner.
- Participate in required staff meetings, supervision, and daily clinical staff meetings.
- Participate in required training, consultation calls, and on-site visits or reviews when required by contract.
- Establish and maintain collaborative working relationships with clinical, administrative, support staff, community partners, clients, and families or natural supports.
- Complete S:US charting requirements, correspondence, and other paperwork accurately and promptly.
- Communicate pertinent consumer and operational information to the team leader and senior management.
- Assist in planning, developing, and implementing goals and training to further develop participants’ skills.
- Report and document all incidents to the team leader within 24 hours.
- Process and sign medication orders, complete medication refills, and obtain prior authorization for specific medications based on provider requests.
- Ensure access to medications through timely refills, obtaining sample medication as ordered, and appropriate use of discretionary funds or patient assistance programs.
- Accept delegated expanded assignments and responsibilities within the S:US system, including quality-improvement teams, task groups, committees, consultations, mentoring, or training.
- Operate as a member of a multidisciplinary team and ensure services are provided in community and clinical settings as required by contract.
- Perform other duties as assigned.
- One year of experience working with adults with severe mental illness.
- Bilingual ability in any language.
