Requirements
  • A bachelor's degree in psychology, social work, sociology, or a related field is required.
  • At least 2 years of experience in case management from a social service agency or serving mental health and/or substance-using populations is required.
  • Knowledge of substance use, mental illness, treatment options, and the needs of individuals integrating back into the community is required.
  • Excellent written, verbal, and computational skills are required to present and document records in accordance with program standards.
Responsibilities
  • Engage recipients during inpatient detoxification or rehabilitation stays, participate in discharge planning, and assess community transition supports needed to stabilize each individual.
  • Provide individual and group support sessions focused on relapse prevention, independent community living, optimum mental and emotional health, addiction and substance-use issues, family and social relationships, stress and symptom management, activities of daily living, medication management, and housing readiness.
  • Work with recipients and housing providers to resolve clinical issues affecting recipients’ ability to obtain, manage, and retain supportive housing.
  • Assist recipients with applying for and navigating Supplemental Security Income, Social Security Disability, Medicaid, and other necessary benefit services.
  • Build relationships and advocate with community providers to connect recipients with appropriate services and expedite and solidify placement in community housing.
  • Provide community navigation, including accompanying recipients to initial behavioral-health and medical appointments, travel training, reengagement in community care, and referrals to services while identifying and addressing barriers to obtaining and maintaining those services.
  • Establish collaborative working relationships with inpatient treatment teams and other partners to plan appropriate recipient discharges.
  • Provide intensive emotional and practical support as recipients transition back into their communities.
  • Provide out-of-hours crisis intervention services to recipients and their support networks, including respite referrals and other diversion and stabilization services.
  • Assess housing supports and provide housing assistance and placement for homeless recipients, including completing HRA 2010e forms, applying for housing, preparing for interviews, following up with housing providers, and assisting with moving in, obtaining housing supplies, and learning the neighborhood.
  • Attend and participate in team meetings and supervisory sessions.
  • Develop short-term, person-centered treatment plans to help recipients achieve recovery-related and other goals.
  • Monitor and record recipients’ progress toward treatment goals.
  • Assist and instruct individuals with personal hygiene, grooming, laundry, nutrition and meal preparation, budgeting, daily living and socialization skills, and maintaining a safe living environment.
  • Complete required in-service training and staff development.
  • Perform other related duties as assigned.
Desired Qualifications
  • A master's degree is preferred.
  • New York State CASAC certification is preferred.
  • Knowledge of treatment, rehabilitation, and community support programs as they relate to recipients and their families is preferred.
  • Knowledge of techniques for identifying risk, including crisis-management techniques, is preferred.
