Requirements
  • Graduate of an approved school of professional nursing.
  • Hold a current Registered Nurse license in the practicing state.
  • Have one or more years of experience and responsibilities in clinical home health services, or equivalent.
  • Hold a valid driver's license in the appropriate state and have independent transportation with current insurance coverage.
  • Demonstrate problem-solving ability and organizational skills while requiring minimal direction.
  • Demonstrate competence in applying processes that ensure adherence to pertinent federal and state rules and regulations.
  • Communicate clearly and effectively with staff, physicians, referral sources, patients, and their families.
  • Demonstrate computer literacy skills.
Responsibilities
  • Follow the appropriate care plan in collaboration with the Director of Nursing, patient or client, caregivers, physician, and other care team members as appropriate.
  • Perform on-site orientation, quarterly supervision visits, skill checks, and annual performance evaluations of home-care aides in compliance with applicable regulations and company policies.
  • Provide ongoing instructions to home health aides during field visits, including reinforcement of standard precautions, client respect, HIPAA, and safety.
  • Review personal care plans from Aging Service Access Points and orient aides in following the care plan as authorized by the Aging Service Access Point registered nurse.
  • Document services rendered and changes in client conditions or family and home situations.
  • Communicate with Aging Service Access Point case managers or the client's legal representative or family about concerns or issues regarding client status as necessary.
  • Treat patients when appropriate and as needed for skilled nursing services.
  • Administer the care plan and support the patient's or client's health and wellness as indicated by the care plan.
  • Assure compliance with nursing and clinical standards, government regulations, and company policies.
Desired Qualifications
  • Current CPR certification.
  • Equivalent experience to one or more years of clinical home health services experience.
