- 1 year of case management experience or related experience. Or an equivalent combination of related education and experience.
- Provide group or individual job retention, maintenance support and counseling.
- Assist participants with issues pertaining to work-related benefits and transitional benefits.
- Research and verify 30-day to 90-day, and 180-day via fax, site visits, mail, and in-person appointments.
- Maintain on-going contact with participants to identify any personal and social problems that may arise while being employed and make appropriate referrals for the resolution.
- Facilitate group retention activities in order to help educate participants on benefits and work support as well as career advancement exploration.
- Maintain contact with employers to verify employment, resolve any issues with their WeCARE participant workers, and ensure overall satisfaction with WeCARE relations.
- Coordinate with job developers and Strategic Sector Lead teams to develop and maintain job retention information with employers on working participants.
- Offer employment counseling and referrals to support services or additional educational programs.
- Perform other related duties as assigned.
- Bachelor’s Degree in Vocational Rehabilitation Counseling or another related field preferred.
- Bi-lingual (English Spanish) candidates preferred.
