Requirements
  • A Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program.
  • A current, unencumbered, active license to practice as an LPN/LVN in the state.
  • The ability to read, write, speak, and understand English.
  • The ability to read technical procedures and comprehend policy and procedure manuals.
  • The ability to apply fractions, percentages, ratios, and proportions to practical situations.
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of residents.

Responsibilities
  • Provide direct nursing care to residents.
  • Supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Admit, transfer, and discharge residents, including completing associated records and forms.
  • Complete accident and incident reports and chart related events.
  • Maintain resident census reports and forward charge slips to the Business Office.
  • Complete required medical forms, reports, evaluations, studies, and charting.
  • Protect resident health information and facility information-system access credentials, and report suspected unauthorized access or disclosure.
  • Complete recordkeeping forms and charts upon resident admission, transfer, and discharge.
  • Receive and document telephone orders from physicians and transcribe orders to resident records, medication cards, and treatment or care plans.
  • Chart nurses’ notes and sign and date all entries in resident medical records.
  • Record diet orders and report discrepancies in physician orders, diet changes, or charting.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, supply, and equipment inventories and report discrepancies or shortages.
  • Review medication cards for completeness, accurate transcription, and compliance with stop-order policies.
  • Notify physicians about automatic stop orders before the final dosage and dispose of medications and narcotics according to procedures.
  • Participate in employee performance evaluations, determine shift staffing needs, and recommend personnel actions to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and conduct daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Provide leadership to nursing personnel assigned to the unit or shift.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that trainees receive direct licensed-nurse supervision.
  • Meet regularly with nursing personnel to identify and correct problems and improve services.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Report occupational exposures, suspected fraud, and other incidents according to facility procedures.
  • Inform nursing personnel about new admissions and prepare rooms for incoming residents.
  • Greet and orient new residents and family members to the facility.
  • Make physician rounds and arrange diagnostic and therapeutic services as ordered.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts for treatments, medication orders, diets, and other care requirements.
  • Implement nursing objectives and standards and verify that prescribed treatments are administered properly.
  • Monitor residents’ physical and emotional status, including seriously ill residents.
  • Provide or oversee professional nursing services such as catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital-sign measurement, and range-of-motion exercises.
  • Apply restraints when necessary and according to established procedures.
  • Notify physicians and next of kin about accidents, incidents, and changes in resident condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect and maintain nursing treatment areas in a clean and safe condition.
  • Ensure direct care follows resident care plans and wishes, and that residents unable to call for help are checked frequently.
  • Meet with residents and family members and report problem areas to the Nurse Supervisor.
  • Arrange transportation and escort discharged residents to the pickup area.
  • Inform family members of a resident’s death, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training classes.
  • Orient new employees to the shift, policies, procedures, job duties, and work methods.
  • Assist with clinical supervision for nurse aide trainees and attend required continuing education and facility training.
  • Monitor safety practices, infection-control procedures, hand hygiene, personal protective equipment use, and emergency drills.
  • Maintain clean and sanitary work areas and ensure appropriate supplies, equipment, and material safety data sheets are available.
  • Recommend and maintain equipment and supply stock levels and ensure equipment is operated safely by trained personnel.
  • Review resident care plans, identify needed revisions, and ensure nursing notes document adherence to care plans.
  • Assist with Minimum Data Set assessments and care-plan scheduling, revision, and implementation.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman.
