Requirements
  • New York State Licensure as a Licensed Master Social Worker is required.
  • A Master of Social Work degree from an accredited School of Social Work program is required.
  • At least one year as a graduate student or as a social worker with clinical social work experience, including experience in care transitions, is required.
  • Ability to create collegial and collaborative relationships internally and externally.
  • Knowledge of accreditation standards, health care regulations, performance improvement, and patient safety.
  • Effective organization, oral and written communication, problem-solving, program development, and computer skills.
  • Knowledge of health care law and regulations related to acute care and the immediate post-acute continuum.
  • Ability to complete psychosocial assessments, risk factor analyses, treatment planning, and intervention models.
Responsibilities
  • Create optimal outcomes for patients and families by managing complex psychosocial and economic comorbidities, mobilizing resources to reduce risk, and serving as an ambassador between the hospital and community.
  • Work with the Case Manager and treatment team to develop safe and timely Discharge Plans A and B that comply with regulations and laws regarding patient and family wishes.
  • Identify the need for and conduct family meetings that result in comfort, decisions, and other important outcomes.
  • Lead in obtaining charity and financial resources, legal guardianship, adoptions, psychiatric referrals, and competency determinations.
  • Lead or co-lead Care Coordination Rounds.
  • Help create function and productivity on the teams to which the social worker belongs by facilitating conflict resolution and presenting a united message to patients and families.
  • Acknowledge diversity issues and adjust practice accordingly.
  • Work with community people and agencies to improve the responsiveness, capabilities, alignment, and evaluation of services to patients and families.
  • Collaborate with Risk Management, Patient Relations, Utilization Management, and Quality Management regarding potential problems and prevention of detrimental outcomes.
  • Base individual practice patterns on social work principles and methodologies, social work policies, and the NASW Code of Ethics.
  • Provide department coverage as needed, including possible modifications to the role, responsibilities, and assigned caseload.
  • Complete initial patient and family screening and comprehensive psychosocial assessments of patients.
  • Assist patients and families in understanding their illnesses, treatment options, and the consequences of treatment or refusal of treatment.
  • Assist patients and families in communicating with the treatment team.
  • Provide crisis intervention.
  • Advocate for patient care and timely discharge planning.
  • Coordinate patient discharge and continuity-of-care planning.
  • Educate hospital staff about patient psychosocial needs.
  • Educate patients and families about levels of care, including acute, sub-acute, community services, and resources.
